摘要
目的探讨急性脑梗塞患者血清中促甲状腺激素(thyroid-stimulating hormone,TSH)的含量变化及临床意义。方法回顾性分析2010年3月-2012年3月我院神经内科收治的30例急性脑梗塞合并亚临床甲状腺功能减退(subclinicalhypothyroidism,SCH)患者的临床及实验室资料,对其神经功能缺损程度进行NIHSS评分,用酶标免疫方法进行急性期甲状腺激素TSH、游离甲状腺素(free thyroxin,FT4)、游离三碘甲状腺原氨酸(free triiodothyronine,FT3)测定,将TSH升高值分为轻度及中度两组,并比较TSH不同程度升高时NIHSS评分变化。结果 TSH中度升高组NIHSS评分明显低于轻度升高组,两组间差异有统计学意义(P<0.05)。结论急性脑梗塞时神经功能缺损程度受TSH水平变化的影响,TSH水平越高,神经功能缺损程度相对越轻,TSH水平对判断病情程度有一定的参考价值。
[ Objective ] To investigate the influence of serum thyroid-stimulating hormone(TSH) on the neurological impairment of acute ischemic stroke patients . [ Methods ] A retrospective study of the clinical characteristics and laboratory data of the acute ischemie stroke with subclinical hypothyroidism (SCH)patients, who were admitted from March 2010 to March 2012 by our hospital. Neurologcal impairment was assessed using NIHSS, TSH, FT4 and FT3 were measured by RIA(radioimmunoassay). We dvidcd all patients into 2 groups with mild and moderate based on the TSH levels. All values were compared between the NIHSS scores with TSH level. [ Results ] The NIHSS score of the moderate group was significantly lower than the mild group, there was significant difference between the two groups (P 〈0.05). [ Conclusion ] The severity of neurologcal impairment associated with the TSH levels, neurologcal impairment is relatively lighter when TSH level higher .The TSH level has some value to predict the severity of neurologcal impairment.
