摘要
目的观察乙酰肝素酶(Hpa)在乳腺癌的表达,探讨它们的表达与乳腺癌浸润、转移的关系及临床意义。方法对60例未行任何化疗、放疗和内分泌治疗的原发性乳腺癌病理标本进行分析,应用苏木素-伊红(HE)染色观察乳腺癌的组织形态和病理学变化;应用免疫组织化学法检测乳腺癌患者癌组织、癌旁2cm乳腺组织及距癌组织5em以上的正常乳腺组织中Hpa的表达。结果60例乳腺癌病理标本免疫组织化学结果显示,乳腺癌组织Hpa阳性表达率为68.33%(41/60),癌旁组织(癌旁2cm)Hpa阳性表达率40.00%(24/60),正常组织Hpa阳性表达率0.00%(O/60)。统计分析结果显示,与正常对照组织比较,Hpa在癌组织和癌旁组织表达升高且差异有统计学意义(P〈0.01)。Hpa的表达与乳腺癌临床病理标志的相互关系分析结果显示:Hpa的表达与乳腺癌患者体内瘤块直径、组织学分级、腋下淋巴结状况及临床分期明显相关(P〈0.05),与患者年龄无明显相关(P〉0.05)。而且具有腋下淋巴结转移的患者Hpa阳性表达率显著高于无腋下淋巴结转移组(P〈0.01)。结论Hpa在乳腺癌中的表达均较高,明显高于癌旁组织及正常乳腺组织;Hpa的表达与乳腺癌肿瘤大小、组织学分级、临床分期、淋巴结转移密切相关,与患者的年龄无明显相关。
Objective To study the expression of heparanase (Hpa) in patients with breast canc- ers, and investigate the relationship between Hpa expression with tumor infiltration and metastasis, and clinical significance. Methods Sixty cases of age in 32-73 years did not do any chemotherapy, radiation therapy, and endocrine treatment of primary breast cancer pathological specimens for analysis, Morphological and pathological alterations were detected by using hematoxylin and eosin (HE) staining. The expression of Hpa were measured byimmunohistochemical method in the breast cancer tissues, the para tissues 2 cm from the margin of breast cancer tissues and the para tissues 5 cm from the margin of breast cancer tissues. Re- suits The immunohistochemical results showed that, Hpa was positive expression in the cancer tissues from 41 patients of 60 patients and the positive rate was 68.33% (41/60) ;Hpa positive rate was 40. 00% (24/ 60) in the para tissues (2 cm from the margin of cancer tissues) from 60 patients ; Hpa was negatively ex- pressed in the normal tissues, t test showed that compared with the control (normal tissues) , Hpa increased in cancer tissues and para tissue were significant ( P 〈 0. 01 ). The analysis of the inter-relationship between Hpa expression levels and clinical pathological characteristics showed that, Hpa expression levels were relat- ed with the tumor diameter in the patients' body, histological grade, and the migration status of lymph node under armpit (P 〈 0. 05 ). But, there was no significant rlation between Hpa expression level and patient' s ages (P 〉 O. 05). Hpa positive rate was higher in the tissues from the breast cancer patients with armpit lymph node metastasis than that in the tissues from the breast cancer patients without armpit lymph node metastasis ( P 〈 0. 05 ). Conclusion The expression of Hpa up-regulated in the breast cancer tissues. The expression level in the cancer tissues was significantly higher than that in the para canc
