摘要
目的对比观察酒石酸布托啡诺与酮咯酸氨丁三醇用于骨科膝关节周围手术术后镇痛的效果。方法选择膝关节周围下肢手术60例,随机分为2组,每组30例。酒石酸布托啡诺组给予酒石酸布托啡诺10 mg;酮咯酸氨丁三醇组给予酮咯酸氨丁三醇150 mg;2组均用生理盐水稀释至100 mL,手术结束前30 min接静脉自控镇痛泵。观察记录术后1、4、8、12、24、48 h的静态和动态视觉模拟量表(VAS)评分,自控镇痛按压次数,镇痛满意度及不良反应。结果 2组患者术后各时点的静态VAS评分及按压次数比较差异无统计学意义(P>0.05)。术后8、12、24 h的动态VAS评分酒石酸布托啡诺组明显高于酮咯酸氨丁三醇组(P<0.05);酒石酸布托啡诺组恶心、呕吐总发生率高于酮咯酸氨丁三醇组(P<0.05)。结论与酒石酸布托啡诺相比,酮咯酸氨丁三醇用于膝关节周围手术术后镇痛效果好,且不良反应少,是术后镇痛的良好选择。
Objective To compare the analgesia efficacy of ketorolac tromthamine and butorphanol tartrate injection on relieving postoperative pain after knee operation.Methods A total of 60 patients undergoing selective knee operation were randomly divided into two groups,30 cases in each group.The butorphanol tartrate group was given butorphanol tartrate injection（10 mg） with saline 90 mL altogether;the ketorolac tromthamine group was treated with ketorolac tromthamine（150 mg） with saline 95 mL altogether.They were linked to patient controlled intravenous analgesia pump 30 minutes before the operation over.The visual analog scale（VAS） scores of rest pain and dynamic pain,press times,satisfaction degree of analgesia and adverse reaction were observed within 48 hours（1,4,8,12,24,48 h） after the operation.Results The VAS score of rest pain and satisfaction scale showed no significant difference between the two groups（P〉0.05）.The VAS scores of dynamic pain were higher in the butorphanol tartrate group than those in the ketorolac tromthamine group at 8,12,24 h after operation（P〈0.05）.The incidence of nausea and vomiting was higher in the butorphanol tartrate group than that in the ketorolac tromthamine group（P〈0.05）.Conclusion Comparing with butorphanol tartrate injection,ketorolac tromthamine produced fewer side effects in relieving postoperative pain after knee operation,and the analgesia efficacy is satisfying.It is a better choice for postoperative analgesia.
