摘要
目的探讨老年痴呆尿毒症患者行维持性血液透析(MHD)的护理对策。并与未接受心理干预护理的尿毒症患者心理反应进行对比,从而为减轻尿毒症患者的心理负担,提高患者的治疗效果及生活质量提供依据。方法回顾性分析我院2008年1月至2012年1月收治入院的老年尿毒症患者200例临床资料。并对血液透析患者护理干预和未进行干预护理各100例进行效果比对分析。结果所有患者内瘘管及穿刺静脉血管保护良好,生活基本能自理,能顺利完成维持性血液透析的全过程。观察组1年、3年以上存活90例(90.0%)、88(88.0%),对照组75(75.0%)、64(64%),两组存活率比较差异明显,P<0.05;观察组透析低血压、感染、心血管并发症、心力衰竭明显少于未接受护理的对照组,两组比较差异明显,P<0.05;前者能积极主动配合护理工作,获得了较好的透析效果。结论做好细致的围透析期心理护理,提供亲切、舒适、安全的透析环境,严密观察病情的变化,采用恰当的护理措施,满足患者的基本需求,是老年痴呆尿毒症患者顺利完成维持性血液透析的保证。
Objective To study the dementia uremic patient line maintenance hemodialysis （MHD） nursing countermeasures.And did not accept and psychological intervention nursing uremic patients psychological reaction is compared,and thus to reduce a uremic patients psychological burden,the treatment effect of the patients a better quality of life and provide the basis： Methods Retrospective analysis in January 2008 to January 2012 elderly uremic patient was treated 200 patients with clinical data.And the blood dialysis patients nursing intervention and did not intervene in the 100 cases nursing effect than analysis. Results All patients fistula and puncture veins in protection is good,life basic can provide for oneself, can finish the whole process of the maintenance hemodialysis.The observation group 1 year,3 years for 90 eases （90.0%）,88 （88.0%） and control group in 75 （75.0%）,64 （64%）,two groups of survival comparative differences apparent, P〈0.05;The observation group dialysis low blood pressure, infection, cardiovascular complications,heart failure is less than that of the control group did not receive care,two groups of comparisons difference obvious, P〈0.05;The former can actively assist nursing work,obtained the good dialysis effect. Conclusion Do meticulous wai dialysis period psychological care,offers cordial,comfortable and safe dialysis environment,close observation of the change of the illness,and implementation of appropriate care measures to meet the basic needs of patients,is dementia uremic patient successfully completed maintenance hemodialysis guarantee.
