摘要
目的分析近几年宝鸡市中医医院高血压药物的临床应用情况,对合理性进行分析。方法随机选择该院2009-2011年1 860份高血压患者的处方进行统计分析,观察抗高血压药物的常用种类及使用情况。结果临床使用最多的是钙离子拮抗剂,其排列顺位是钙离子拮抗剂(CCB)44.14%、β-受体阻滞剂21.4%、ACEI17.26%、ARB6.67%、利尿剂5.49%和复方制剂4.95%。CCB使用率明显高于其它药物(P<0.05)。使用率最高的两种药物是硝苯地平控释片和氨氯地平片,使用率大大高于其它降压药(P<0.05)。1 860份处方中共发现23份,占1.26%,分别为重复用药18份,剂量偏大2份,用药不合理1份,剂量偏小2份。结论高血压用药应根据指南要求个体化联合用药,总体来看,该院高血压用药基本合理,但还应加强临床用药指导,降低不合理用药。
Objective To analyze the clinical application situation of antihypertensive drugs and the rationality of drug usage in Baoji Hospital of Traditional Chinese Medicine in recent years.Methods The prescriptions of 1,860 hypertensive patients in Baoji Hospital of Traditional Chinese Medicine from 2009 to 2011 were randomly selected for statistical analysis.The common categories and the use situation of antihypertensive drugs were observed.Results Calcium antagonists were widely used in the clinical setting for hypertension,which included calcium channel blockers（44.14%）,beta-receptor blockers（21.4%）,ACEI（17.26%）,ARB（6.67%）,diuretics（5.49%） and compound preparations（4.95%）.The usage rate of CCBs was significantly higher than those of other drugs（P〈0.05）.Nifedipine controlled-release tablets and amlodipine were the two drugs with the highest utilization rates,and the rates were significantly higher than those of other antihypertensive drugs（P〈0.05）.23（1.26%） cases of irrational drug use were found in 1,860 prescriptions,including 18 prescriptions with repeat medication,2 with high-dosage therapy,1 with unreasonable therapy,and 2 with low-dosage therapy.Conclusions The medication for hypertension should be guided by the requirements of ＂the Guidelines for Prevention and Treatment of Hypertension in China＂ and individualized combination therapy may be the best choice.On the whole,medication for hypertension in Baoji Hospital of Traditional Chinese Medicine is basically rational,but it is still necessary to strengthen the guidance for clinical drug use and reduce the irrational drug use.
