摘要
目的分析海南医学院附属医院鲍曼不动杆菌的感染分布与耐药情况,为临床诊治提供依据。方法对10 368例住院患者采集痰液、尿液标本,按照《全国临床检验操作规程》(第3版)要求对痰液、尿液标本进行处理。并采用全自动微生物分析仪及配套试剂进行细菌鉴别和药敏试验。结果 10 368例住院患者共培养分离出鲍曼不动杆菌136株。136株鲍曼不动杆菌病区分布居前5位的是:ICU、神经内科、呼吸内科、神经外科、儿科。136株鲍曼不动杆菌对抗生素敏感的前5位是:多黏菌素B、美满霉素、头孢哌酮/舒巴坦、亚胺培南、环丙沙星。对抗生素耐药依次为:黏菌素、诺氟沙星>头孢曲松>头孢噻肟>庆大霉素>复方新诺明、四环素>氨苄西林/舒巴坦>哌拉西林>阿米卡星>头孢他啶>环丙沙星>头孢吡肟>哌拉西林/他唑巴坦>左氧氟沙星>美罗培南>多黏菌素B>亚胺培南>美满霉素>头孢哌酮/舒巴坦。结论鲍曼不动杆菌对多种抗生素有多重耐药性,头孢哌酮/舒巴坦等可以作为首选治疗的药物。医院应加强对该菌的检测,降低院内感染率。
Objective To analyze the infection distribution and drug resistance of Acinetobacter baumannii in the Affiliated Hospital of Hainan Medical College,and to provide a basis for clinical diagnosis and treatment.Methods Sputum and urine specimens were collected from 10 368 inpatients and were treated according to the national clinical laboratory procedures（3rd edition）.Bacterial identification and susceptibility testing were performed by using automated microbial analyzer and related reagents.Results A total of 136 strains of Acinetobacter baumannii were isolated from the 10 368 patients.The top 5 strains were selected from ICU,department of neurology,department of respiratory medicine,department of neurosurgery and department of pediatrics.The top 5 antibiotics of Acinetobacter baumannii were polymyxin B,minocycline,cefoperazone/sulbactam,imipenem,ciprofloxacin.The drug resistance form high to low were colistin/norfloxacin,ceftriaxone,gentamicin,cotrimoxazole,tetracycline,ampicillin/sulbactam,piperacillin,amikacin,ceftazidime,ciprofloxacin,cefepime,piperacillin/tazobactam,levofloxacin,meropenem,polymyxin B,imipenem,minocycline,and cefoperazone/sulbactam.Conclusion Acinetobacter baumannii was multiresistant to polyantibiotic and cefoperazone/sulbactam can be used as the preferred drug.The surveillance of Acinetobacter baumannii should be enhanced to reduce nosocomial infections.
