摘要 目的:通过三维能量多普勒超声定量分析肾内血流的灌注参数和肾功能测值的关系,来了解原发性肾小球疾病所致慢性肾病患者肾脏损害的程度。方法:根据美国肾脏疾病生存质量指导(K/DOQI)指南诊断及分期标准选择原发性肾小球疾病患者60例,与正常对照组30例进行比较,应用彩色多普勒超声观察肾脏的形态结构,显示肾内各分级动脉血流充盈情况,脉冲多普勒显示段动脉及叶间动脉的脉冲多普勒频谱,测量阻力指数(RI),应用三维能量多普勒超声,观察肾脏的血流灌注显像,并利用血流分析技术,计算血管指数(VI)。将肾血流灌注参数与肾功能测值及肾穿刺活检病理结果进行对照。结果:原发性肾小球疾病组在血流灌注显像、各级动脉的阻力指数(RI)及血管指数(VI)与正常对照组比较,存在显著差异,并随着肾功能损害越严重,差异越显著。结论:三维能量多普勒超声对慢性肾病各期肾血流灌注观察,可以为原发性肾小球疾病的早期肾功能损伤评估提供客观依据。 Objective:The 3-Dimension Powerful Doppler Ultrasonic was adopted to analyze quantitatively the relationship between the perfusion indices of blood flow inside the kidney and the measured values of kidney function so as to know the degree of the kidney damage of chronic kidney disease(CKD) patients caused by primary renal glomerular diseases.Methods: 60 cases of primary renal glomerular diseases were involved in the study group according to the diagnosis and staging criteria of Kidney Disease Survival Quality Instruction made by NKF(K/DOQI) compared with 30 cases as the normal control group.The Colorful Doppler Ultrasonic was adopted to observe the morphology and structure of kidney and show the filling of blood flow in various branches of arteries.The pulse Doppler was adopted to show the pulse Doppler spectrum of the segmental and interlobal arteries of kidney.The resistance index(RI) was also detected.The kidney blood perfusion imaging was observed by 3-dimension powerful Doppler ultrasonic and the vessel indices(VI) was calculated by the blood flow analysis technology.The kidney blood flow perfusion parameters were compared with the measured values of kidney function and the biopsy pathology results.Results: Significant difference of blood flow perfusion imaging,resistance indices(RI) and vessel indices(VI) of all levels of arteries existed between the primary kidney glomerular disease group and normal control group while it became more and more apparent as the kidney function was injured more and more severely.Conclusion:The 3-Dimension Powerful Doppler Ultrasonic can observe the kidney blood perfusion at various stages of CKD so that it can provide objective basis for the evaluation of early kidney function injury of primary renal glomerular diseases.
