摘要
目的探讨切开扩创引流并挂线法治疗高位复杂性肛瘘的临床疗效。方法将76例高位复杂性肛瘘患者随机分为观察组与对照组,每组38例。观察组给予切开扩创引流并挂线法治疗,对照组仅给予切开挂线法治疗。比较2组术后治疗效果。结果治疗后观察组肛管静息压、直肠静息压及肛管最大收缩压显著高于对照组,差异有统计学意义(P<0.05);观察组治疗前后肛管静息压、直肠静息压及肛管最大收缩压差异无统计学意义(P>0.05);对照组治疗后肛管静息压、直肠静息压及肛管最大收缩压显著低于治疗前,差异有统计学意义(P<0.05)。观察组患者住院时间、创面愈合时间短于对照组,肛瘘复发率低于对照组,医疗费用少于对照组,差异均有统计学意义(P<0.05)。结论采用切开扩创引流并挂线法治疗高位复杂性肛瘘患者成功率高,肛瘘复发率少,对肛门功能影响小,安全有效,值得临床广泛推广。
Objective To explore the clinical efficacy of incision debridement and drainage and hanging line method in treatment of high complex anal fistula.Methods 76 cases patients with high complex anal fistula were randomly divided into observation group and control group,each of 38 cases.Observation group were treated with incision debridement and drainage and hanging line method treatment.Control group were treated with hanging line method treatment.Compared the clinical efficacy of two groups after treatment.Results After treatment,anal resting pressure,rectal resting pressure and anal canal maximum systolic blood pressure of observation group were significantly higher than those of control group(P〈0.05);Before and after treatment,compared anal resting pressure,rectalresting pressure and anal canal maximum systolic blood pressure of observation group(P〈0.05);After treatment,anal resting pressure,rectal resting pressure and anal canal maximum systolic blood pressure of control group were significantly lower than before treatment(P〈0.05).Length of stay,wound healing time of observation group were shorter than those of control group,anal fistula recurrence rate was lower than that of control group,medical cost was smaller than that of control group,the difference were statistically significant(P〈0.05).Conclusion Incision debridement and drainage and hanging line method in treatment of high complex anal fistula has a high success rate,less the recurrence rate of anal fistula,safe and effective,worthy of wide dissemination.
