新鲜离体人肺中液体清除作用及其影响因素研究

摘要 目的:证实随着肺泡内灌注液的吸收,肺泡灌注液的蛋白浓度逐渐增加;观察β2-受体激动剂特布他林对新鲜离体人肺肺泡上皮细胞液体清除作用的影响;观察特布他林的这种上调作用是否被心得安所抑制。方法:选取42例胸外科住院肺癌患者,经适当的手术方式切除的段或叶肺组织,应用含有不同浓度的蛋白溶液灌注距离癌肿边缘15 cm的肺段,通氧气1 h后抽取灌洗液测量其蛋白浓度,用以检测肺泡液体清除率。结果:通过检测灌洗前后的蛋白浓度改变,测得A组1 h的肺泡液体清除率(ALC)为(3.43±1.52),B组1 h的ALC为(3.43±1.20),C组1 h的ALC为(5.43±1.51),D组1 h的ALC为(5.14±1.68),E组1h的ALC为(3.43±1.51),F组1 h的ALC为(3.43±1.20)。结论:β2-受体兴奋剂特布他林可以明显提高离体人肺的肺泡液体清除率,这种作用与肺泡内蛋白浓度无关且可被心得安部分抑制,特布他林通过缩短达到肺泡液体清除率峰值的时间来提高肺泡液体清除率,故在临床肺水肿患者适当应用β2-受体兴奋剂有利于肺水肿的清除,患者症状的缓解。 Objective This study was designed with the follow three objectives. The first objective was to test the hypothesis that alveolar protein concentration would increase as instilled solution was cleared from the airspace of human lung. The second objective was to observe the effect of β - agonist terbutaline in alveolar liquid clearance in human lung. The third objective was to observe whether propanolol would inhibit the effect of terbutaline on alveolar liquid clearance. Methods 42 example lung cancer patients undertaked suitable surgery.Then lung organization is collected when it was excised by the surgery, different albumin concentration solution pours into lung section which is 15 cm away from cancer edge. Ventilated for one hour, irrigating solution was extracted, then its protein density was measured to observes the alveolar fluid clearance (ALC). Results ALC after 1 hour of group A is (4.58 ± 1.52) ,ALC after 1 hour of group B is (3.43 ± 1.20) ,ALC after 1 hour of group C is (5.43 ± 1.51 ), ALC after 1 hour of group D is (5.14 ± 1.68), ALC after 1 hour of group E is (3.43 ± 1.51 ), ALC after 1 hour of group F is (3.43 ± 1.20). Conclusion Beta 2 - adrenergie agonist tebutaline increase alveolar fluid clearance of reseeted human lung obviously. The effect has nothing to do with albumin concentration and it is inhibited partly by praetolol. Tebutaline increase alveolar fluid clearance throng reducing ALC peak achieved time. Therefor undertaking Beta2 - adrenergie agonist suitably in pulmonary edema patients is a good method to relieve symptom.
