摘要
目的探讨术前护理访视对手术患者术前、术中、术后应激反应(包括焦虑值、血压和心率)的影响,并积极寻找护理对策,促进手术患者术后康复。方法将600例择期行手术的患者分成两组,观察组患者在常规护理的基础上进行术前护理访视,对照组只进行常规护理,观察两组焦虑值、血压和心率的变化,并评价其护理效果。结果观察组患者入手术室15 min、麻醉前、手术开始15 min、术毕时和术后24 h 5个时点的焦虑值、血压和心率与基础值差异均无统计学意义(P>0.05);对照组患者入手术室15 min、麻醉前、手术开始15 min 3个时点的焦虑值与基础值差异有统计学意义(P<0.05),但血压和心率只在入手术室15 min和麻醉前2个时点与基础值差异有统计学意义(P<0.05);观察组患者对手术室护理服务的满意度明显高于对照组(P<0.05)。结论术前护理访视能减轻患者的围术期应激反应,从而使患者顺利渡过手术期,减少不良反应的发生,同时有利于提高护理质量,改善医患关系。
Objective To explore the effect of preoperative nursing supervision on preoperative,intraoperative and postoperative stress response（including anxiety score,blood pressure and heart rate） of surgical patients and to actively explore nursing strategies to promote the rehabilitation of surgical patients.Methods 600 cases of elective surgery patients were assigned to the observation group and the control group.Patients in the observation group was given nursing supervision on the basis of conventional care,and the control group was only given the conventional care.Anxiety score,blood pressure and heart rate of the two groups were observed,and the nursing effects were evaluated in both groups.Results Anxiety scores,blood pressures and heart rates of the observation group at the five time points of 15 min after entering operation room,pre-anesthesia,15 min before surgery,completion of surgery and 24 h after surgery had no significant differences with baseline values（P〈0.05）;however,anxiety scores of the control group at three time points of 15 min after entering operation room,pre-anesthesia,15 min before surgery were significantly different from baseline values（P〈0.05）,blood pressures and heart rates of the control group were significant differences with baseline values at two time points of 15 min after entering operation room and pre-anesthesia（P〈0.05）.Operating room care satisfaction rate was significantly higher in the observation group than the control group（P〈0.05）.Conclusion Preoperative nursing supervison can reduce the patients＇ perioperative stress response,so that patients can undergo surgery period smoothly,reduce the incidence of adverse reactions.Preoperative nursing supervision can also help to improve the quality of care and to improve the doctor-patient＇s relationship.
