摘要
目的提高髋关节置换术围术期质量评价结果的科学性、规范性、可比性。方法运用测量系统分析技术,分析出院病历中的髋关节置换术围术期医疗质量评价分类数据,从一致性比率和卡帕值2方面评价围术期质量评价系统的优劣,并实施针对性改进。结果改进后,髋关节置换术围术期质量评价系统的一致性比率和卡帕值均极大改善。系统总体一致性比率Passessment由65%提高到95%,所有质控员与标准的kappaFleiss由0.82提高到0.97。结论运用测量系统对髋关节置换术围术期质量评价系统进行分析,可迅速、有效地识别影响质量评价的关键因素,并针对性的改进,使围术期医疗质量评价更加科学、全面、规范。
Objective To make the results of hip replacement perioperative quality evaluation more scientific, normative, comparability. Method Using the MSA to analyze the hip replacement perioperative quality evaluation data, and evaluate the effect of the perioperative quality evaluation by the agreement proportion and kappa value, so as to practice directive improvement measures. Result The agreement proportion and kappa value improved greatly the ^-Passess mentappraisers vs standard） increased from 65 % to 95 % （〉 80%）,^-kappa^Fleiss{all inspectors vs standard） was raised from 0.82 to 0.97 （~0.90}. Conclusion MSA can help identify the key factors affecting medical quality evaluation, so as to take specific improvement measures and make the perioperative medical quality evaluation system more scientific, com- prehensive and normative.
