摘要 目的比较单纯内固定(可吸收螺钉或可吸收线)与内固定联合外展支架治疗移位型肱骨大结节骨折的疗效。方法 2007年8月-2010年12月本院41例肱骨大结节骨折患者,22例采用单纯内固定(A组),19例采用内固定联合外展支架(B组)治疗,评估术后肩关节功能Neer评分,分析比较两种手术的疗效。结果 41例患者均通过门诊随访3-12个月,术后A组X线评估22例患者2,例骨折移位2,0例无移位,肩关节功能平均Neer评分84.3分,Neer评分优良率为72.7%,B组X线评估19例,患者骨折均无移位,肩关节功能平均Neer评分92.6分,Neer评分优良率为94.7%,A组与B组疗效比较具有统计学意义(P<0.05)。结论内固定联合外展支架治疗移位型肱骨大结节骨折疗效优于单纯内固定治疗。 [ Objective ] To compare curative effect between internal fixation ( absorbable screws or absorbable sur- gical suture) and internal fixation combined external fixator in treating the Greater Tuberosity Fracture of the Humerus. [ Methods] 41 cases were admitted with acute Greater Tuberosity Fracture of the Humerus from August 2007 to De- cember 2010. 22 cases received internal fixation ( group A) and 19cases received internal fixation combined external fixator. ( group B). Neer score were used to evaluate the post - operative outcome. [ Results ] The post - operative fol- low - up of 41 cases was 3 - 12 months. 2 of 22 cases in group A occurred fracture displacement. The average Neer score in group A was 84.3. The excellent rate of Neer score in group A was 72.7%. None of 19 cases in group B oc- curred fracture displacement. The average Neer score in group B was 92.6. The excellent rate of Neer score in group B was 94.7%. The curative effect between group A and group B had statistical significance (P 〈 0.05 ). [ Conclusion] Internal fixation combined external fixator in treating the Greater Tuberosity Fracture of the Humerus offers better thera- peutic results than using internal fixation alone.
