摘要
目的:探讨系统性红斑狼疮(SLE)患者淋巴细胞VCS参数的变化及临床意义。方法:利用Beckman-Coulter Gen.S血液细胞分析仪的VCS技术,测定79例SLE患者(SLE组活动情况根据SLEDAI积分判断:分为轻度活动组25例、中度活动组28例、重度活动组26例)、85例其他风湿免疫性疾病患者(疾病对照组)及50名健康对照者的WBC、淋巴细胞百分率(LY%)和LY的体积(LYV)、电导率(LYC)、光散射性(LYS),及各自的平均分布宽度(LYVSD、LYCSD、LYSSD),并采用ROC曲线分别评价各参数的敏感度和特异度。结果:参数LYVSD、LYC、LYCSD、LYS在SLE组分别为19.7±5.8、109.9±8.2、13.0±2.8、53.0±8.2,疾病对照组分别为15.9±2.1、112.9±4.4、11.6±2.4、47.7±6.3,健康对照组分别为14.1±1.3、114.3±2.0、10.7±1.3、41.9±2.1,差异有统计学意义(F值分别为34.259、15.244、13.479、23.226,P均<0.05),以SLE组的上述各参数变化最为显著。SLE患者疾病活动期各组的LYV、LYS、LYVSD、LYCSD、LYSSD明显升高,而LY%和LYC显著下降,以重度活动组的LYVSD、LYCSD、LYS变化最为显著(P均<0.05)。ROC曲线分析LYVSD、LYC、LYCSD、LYS的曲线下面积分别为0.748、0.626、0.621、0.633,其中LYVSD的截值(cut-off值)≥15.9时,其敏感度和特异度达80.7%和71.4%。结论:SLE患者的淋巴细胞体积、细胞内部结构及胞浆复杂性等发生了明显变化,以SLE重度活动组的LYVSD、LYCSD、LYS变化最明显;LYVSD(cut-off值≥15.9)在区分SLE和其他风湿免疫性疾病时具有较高的敏感度(80.7%)和特异度(71.4%)。
Objective To explore the significance and changes of peripheral blood lympholeukoeyte VCS parameters in patients with SLE. Methods The WBC count, LYV, LYC, LYS, LYVSD, LYCSD, LYSSD were detected in 79 patients with SLE, 85 patients with immunologieally mediated diseases as rheumatosis and 50 healthy people by VCS technology using blood cells analysis equipment of Beckman-Coulter. And 79 patients with SLE were divided into mild group with 25 eases, moderate group with 28 cases, and severity group with 26 cases. Results The LYVSD, LYC, LYCSD, LYSwere 19.7±5.8, 109.9±8.2, 13.0±2.8, 53.0±8.2 respectively in SLE patients group, were 15.9±2.1, 112.9 ± 4.4, 11.6 ±2.4, 47.7 ± 6.3 in diseases control group. It had significant difference between these two groups (P 〈 0.05 ). The LYV, LYS, LYVSD, LYCSD and LYSSD were increased obviously in SLE patients group compared to the other two groups, especially LYVSD, LYCSD and LYS in severity group ( P 〈 0.05 ). The LY percentage and LYC were decreased in SLE patients group. The area under curve of LYVSD, LYC, LYCSD and LYS were 0.747, 0.626, 0.621 and 0.633 separately by ROC analysis. When the LYVSD cutoff value I〉 15.9, its sensitivity and speeificity were 80.7% and 71.4%. Conclusion The LYV and cell construction changed obviously in SLE patients, especially the LYVSD, LYCSD, LYS in severity group of SLE patients. It had a good diagnostic value in differing SLE patients from other immune diseases patients when LYVSD eutoff value ≥ 15.9.
