钼靶指导乳腺钙化灶切除在发现非肿块型早期乳腺癌中的应用 被引量:12

摘要 目的探讨利用钼靶X线辅助乳腺钙化灶切除活检在发现非肿块型乳腺癌中的应用价值。方法阳江市人民医院乳腺科2009年12月~2010年12月对75例临床触诊阴性乳腺钙化灶在钼靶指导辅助下进行切除活检手术,通过回顾性分析来评价其在发现非肿块型早期乳腺癌乳腺中的应用价值。结果术后病理学诊断:浸润性导管癌5例,导管内癌15例,不典型增生20例,乳头状瘤24例,余均为良性。5例浸润性导管癌接受外科手术及辅助治疗,15例导管内癌接受单纯乳房切除,该组患者均为0~Ⅰ期,治疗后随访1年均未有复发转移。20例不典型增生及24例乳头状瘤接受外科开放扩大切除病灶手术,该组患者随访1年,被切除病灶无复发征象。结论利用钼靶X线辅助作乳腺钙化灶切除活检能准确发现非肿块型乳腺病灶,包括早期乳腺癌、癌前病变,在提高乳腺癌早期诊断率中意义重大。 Objective To explore value of the breast calcification foci resection guided by Molybdenum target mammography for findingnon-mass-application of early stage breast cancer. Methods From December 2009 to December 2010,75 patients with nonpalpable breast microcalcifications in molybdenum target instruction assisted excisional biopsy was performed operation,through a retrospective analysis to evaluate its application value for the non mass in early breast cancer. Results In postoperative pathological diagnosis,5 cases were invasive duct carcinoma, 15 cases were ductal carcinoma,20cases were atypical hyperplasia,24 cases were papillomas.The rest are benign lesions.5 cases of invasive ductal carcinoma were treated by surgical operation and adjuvant therapy,15 cases of ductal carcinoma underwent simple breast resection,the patient group showed 0-I stage,treatment follow up 1 years without recurrence and metastasis.20 cases of atypical hyperplasiaand 24 cases of papillary tumors underwent surgical extended resection operation,the group of patients followed up for 1 years,was excised lesions without recurrence. Conclusion Resection of breast calcification foci guided by Molybdenum target mammography could accurately find a lump breast lesions,including early breast cancer,precancerous lesions.It is important in improving the early diagnosis rate of breast cancer.
