摘要
目的探讨测定心电图QRS波时限、血浆脑钠肽(BNP)及超敏C-反应蛋白(hs—CRP)水平对慢性心力衰竭诊断的价值。方法按2009年ACCF/AHA心力衰竭指南诊断标准选择慢性心力衰竭(CHF)患者150例,非心力衰竭(对照组)45例。所有入选患者均测定QRS波时限及检测血浆BNP、hs-CRP水平,并进行纽约心脏病协会(NYHA)心功能分级和左室射血分数(LVEF)测定。结果CHF组QRS波时限、血浆BNP、hs—CRP水平均明显高于对照组,并随NYHA心功能分级增加而上升(P〈0.05)。CHF组QRS波时限、血浆BNP与NYHA心功能分级呈显著正相关(P均小于0.05),与LVEF呈显著负相关(P均小于0.05);血浆hs—CRP与NYHA心功能分级及LVEF无明显相关(P〉0.05)。结论心力衰竭患者QRS波时限和血浆BNP及hs—CRP随心力衰竭程度的加重而相应升高,QRS波时限及血浆BNP与左心功能状态有良好的相关性,对判断心力衰竭患者心功能及指导治疗有较高价值。
Objective To investigate the diagnostic values of measurements of QRS complex duration, plasma B-type natriuretic peptide (BNP) and high sensitive C reactive protein (hs-CRP)in patients with chronic heart failure. Methods A total of 150 patients with chronic heart failure were admitted, and 45 patients without heart failure were choosed as controls. QRS complex duration, plasma BNP, hs-CRP concentration, left veutricular ejection fraction (LVEF) and NYHA functional classification of the patients were measured. Results The QRS complex duration, and the plasma levels of BNP and hs- CRP were higher in CHF group than those in the control group (P 〈 0. 05) and were elevated with the increase of the NYHA functional classification. The QRS complex duration and plasma levels of BNP were correlated positively with NYHA functional classification in CHF ( P 〈 0. 05 ) and were correlated nega- tively with LVEF. There was no relationship between plasma hs-CRP and NYHA functional classification, LVEF (P 〉 0. 05 ). Conclusions QRS complex duration and the levels of plasma BNP and hs-CRP concentration are increased with severity of NYHA classification. QRS complex duration and plasma BNP are related more closely to heart function, and were effective biological parameters for assessing heart dys- function and guiding treatment in CHF.
