急性有机磷农药中毒性肠坏死的CT表现

摘要 目的探讨急性有机磷农药中毒(AOPI)合并肠坏死的CT诊断价值。方法将68例AOPI患者按其临床诊断及分级标准分为3组:轻度中毒组(27例)、中度中毒组(20例)、重度中毒组(21例),并进行CT检查。结果重度中毒组:肠腔扩张积液15例,肠壁增厚3例,薄纸样肠壁改变6例,肠壁积气6例,门脉积气6例,肠壁密度减低3例,胃壁积气3例,胸腔、腹腔积液2例,双肾及脾脏内多发气泡影1例。中度中毒组显示肠腔扩张积液6例,肠壁未见增厚及密度的改变,门脉系统未见异常。轻度中毒组所有影像学检查未见肠壁及肠腔明显异常改变,门脉系统未见异常。结论腹部CT能够反应重度有机磷农药中毒所致的肠损害,是一种诊断肠坏死的快速、敏感、可靠及无创的影像学方法。 Objective To explore the CT manifestations of intestinal necrosis induced by acute organophosphorus pesticide intoxication(AOPI).Methods 68 cases of AOPI were divided into 3 groups the mild poisoning group of 27 cases,the moderate poisoning group of 20 cases and the severe group of 21 cases,and CT examinations were performed.Results In the severe poisoning group,intestinal cavity expansion was found in 15 cases,wall thickening in 3 cases,thin paper-like wall in 6 cases,intestinal wall pneumatosis in 6 cases,portal pneumatosis in 6 cases,intestinal wall density decrease in 3 cases,gastric wall pneumatosis in 3 cases,pleural and peritoneal effusion in 2 cases and bilateral renal and splenic multiple bubblesin one case.In the moderate poisoning group,intestinal cavity expansion was found in 6 cases.In the mild poisoning group,no abnormal changes were found in the intestinal wall and cavity or the portal system.Conclusion Abdominal CT scanning can be used to diagnose the intestinal injury induced by AOPI.
