摘要
目的评价国产材料软壳技术在糖尿病患者白内障超声乳化手术中的疗效和安全性。方法糖尿病伴白内障患者35例(18例男性,17例女性,共47眼)随机分为国产组(黏弹剂联合采用甲基纤维素和透明质酸钠)24眼、进口组(黏弹剂采用Alcon公司的DuoVisc)23眼。分别在术前、术后1d、1周、1个月测量两组受试者角膜内皮计数和角膜厚度。结果两组受试者术后角膜内皮计数显著低于术前(F=128.2,P<0.01),但国产组和进口组之间没有显著差异(F=1.4,P=0.24)。术后国产组和进口组角膜内皮细胞丢失率分别为:术后1d 10.3%、7.3%,术后1周6.2%、5.8%,术后1个月5.8%、5.5%。两组受试者术后角膜厚度显著增加(F=42.3,P<0.01)。国产组和进口组之间亦没有显著差异(F=1.9,P=0.18)。但采用t检验进行分析,发现国产组在术后1d和1周时角膜厚度显著大于进口组(t=2.9、3.1,P=0.005、0.004),而术后1个月,两组之间角膜厚度无显著性差异(t=0.7,P=0.50)。结论对于糖尿病合并白内障患者,国产材料软壳技术在术后早期效果不及进口软壳技术,但远期效果与进口材料相一致,同样具有保护角膜内皮的效果和良好的安全性。
Objective To evaluate the efficacy and safety of domestic soft shell material technology in diabetic patients with pha- coemulsification. Methods Thirty-five diabetes patients with cataract (18 male, 17 female,47 eyes)were randomly divided into domestic group(sodium Tvlose and sodium hyaluronate) with 24 eyes,and import group(Viscoat+ProVisc) 23 eyes. Respectively,before surgery, one day,one week,and one month after surgery,two groups of subjects were measured corneal thickness and corneal endothelial count. Results The results of two groups of subjects corneal endothelial count were significantly lower than preoperative (F=128.2,P〈0.01), but there were no significantly difference between two groups(F=1.4,P=0.24). Corneal endothelial cell loss rates of domestic group and imported group were respectively:after one day 10.3%,7.3%;after a week 6.2%,5.8%;after one month 5.8%,5.5%. Corneal thickness of both groups increased significantly (F=42.3,P〈0.01). Domestic group and imported group had no significant difference(F=1.9,P=0.18). However,t-test analysis,found that domestic group corneal thickness was significantly greater than the import group one day and one week after surgery(t=2.9,3.1,P=-0.005,0.004),and after one month,corneal thickness between the two groups were no significant difference(t=0.7,P=0.50). Conclusion For patients with diabetic cataract,domestic materials soft-shell technology is less effective than im- ported soft-shell technology in the early postoperative period,but the long-term effect of domestic materials is consistent with the imported materials,which has the same protective effect of the corneal endothelium and good security.
