摘要
目的研究局部亚低温治疗对急性脑出血患者血清基质金属蛋白酶(MMP-9)和细胞纤维连接蛋白(cellularfib ronectin,c-Fn)的影响。方法随机将80例脑出血患者分为局部亚低温组(40例)和常规治疗组(40例),同时设健康对照组23例;常规治疗组应用药物治疗,局部亚低温组在药物治疗基础上加用局部亚低温治疗,观察两组患者发病第1、3、7d血清MMP-9和c-Fn水平的变化。结果两组患者发病第1d血清MMP-9和c-Fn水平均高于健康对照组(P<0.01);第3d常规治疗组MMP-9和c-Fn水平达高峰,局部亚低温组下降,明显低于常规治疗组(P<0.05);在发病第7d,局部亚低温组血清MMP-9和c-Fn仍明显低于常规治疗组(P<0.05)。结论颅脑局部亚低温治疗可显著降低脑出血后患者血清MMP-9和c-Fn的浓度,减轻脑出血后的炎症反应,发挥其脑保护作用。
Objective To study the impact of local mild hypothermia on serum serum matrix metalloproteinase(MMP-9) and cell fibronectin(c-Fn) in patients with acute intracerebral hemorrage.Methods Eighty patients with acute cerebral hemorrage were randomly divided into local mild hypothermia group(40 cases) and routine therapy group(40 cases).Meanwhile,there was a control group of twenty-three healthy cases.Conventional drugs were given to routine therapy group while the mild hypothermia group was given local mild hypothermia treatment but for conventional therapy.Serum levels of MMP-9 and c-Fn in patients were determined dynamically on the 1st,3rd and 7th day after paroxysm.Results On the 1st day after paroxysm,the serum levels of MMP-9 and c-Fn in patients with intracerebral hemorrhage were apparently higher than the normal controls(P0.01).On the 3rd day,the levels of MMP-9 and c-Fn were significantly decreased in local mild hypothermia group in comparison with routine therapy group(P0.05).On the 7th day after paroxysm,the levels of MMP-9 and c-Fn in local mild hypothermia group were still significantly lower than that of routine therapy group(P0.05).Conclusion The local mild hypothermia therapy can not only dramatically reduce the serum levels of MMP-9 and c-Fn but also inhibit inflammatory reaction so as to protect neurological function in patients with cerebral hemorrhage.
