摘要
目的探讨帕金森病(PD)患者疾病的经济负担及其相关影响因素。方法对150例PD患者进行问卷调查,了解患者及其家庭的基本资料和2009年有关PD支出的费用(包括直接费用、间接费用)。应用Hoehn和Yahr分级(H-Y分级)量表评定PD患者的病情。应用单因素分析和多元线性逐步回归分析判断影响PD患者疾病经济负担的相关因素。结果 PD患者的年平均PD支出为13576.43元,其中直接费用为12 306.73元(90.6%),间接费用为1269.70元(9.4%);分别占年家庭收入和人均收入的17.9%和48.0%。单因素分析发现,PD患者的门诊次数(χ2=6.423,P<0.05)及家庭月收入(χ2=13.718,P<0.05)对疾病经济负担的影响有统计学意义。多元逐步回归分析显示,PD患者的病情严重程度(t=2.210,P=0.029)、门诊次数(t=2.185,P=0.031)和家庭月收入(t=1.679,P=0.096)为影响PD经济负担的相关因素。结论 PD患者的疾病经济负担较重;而患者的病情严重程度、门诊次数和家庭收入是影响PD经济负担的相关因素。
Objective To investigate the economic burden of the disease and its related influence factors in patients with Parkinson disease （PD）. Methods The basic material of 150 PD patients, their family and their spending on PD in 2009, including direct costs and indirect costs were collected by questionnaiea survey. The condition of PD patients was assessed by the Hoehn-Yahr （H-Y） grading scale. The related influence factors of economic burden on disease in PD patients were judged with the single factor analysis and multiple linear stepwise regression analysis. Results The annual total cost of PD patient on the disease was 13 576.43 Yuan per capita, of which direct cost was 12306.73 Yuan, 90.6% of the total cost, indirect cost was 1269.70 Yuan, 9.4% of the total cost. The annual total cost per capita accounted for the annual income per household and annual revenue per capita was 17.9% and 48.0% respectively. The single factor analysis showed that the frequency of using outpatient service （X2 = 6. 423, P 〈 0. 05 ） and household month income （X2 = 13. 718, P 〈 0. 05 ） were the influence factors, which significantly affected the economic burden. Multiple stepwise regression analyses suggested that patient acuity （t = 2. 210, P = 0. 029 ） , frequency of using outpatient service （ t = 2. 185, P = 0. 031 ） and household month income （ t = 1. 679, P = 0. 096 ） were significantly impacted the economic burden. Conclusions The PD patients have the heavier economic burden. The condition of PD patient, the frequency of using outpatient service and household income are the related influence factors of economic burden on disease.
