摘要
目的探讨CT灌注成像(CTPI)鉴别腋窝炎性增生性与转移性淋巴结的价值。方法将14只健康雌性新西兰大白兔随机平均分为2组,分别建立腋窝炎性增生性淋巴结模型和VX2种植性乳腺癌腋窝转移性淋巴结模型,建模成功后行CTPI。分析两组淋巴结病变的时间-密度曲线(TDC)、各CT灌注参数包括血流量(BF)、血容量(BV)、平均通过时间(MTT)、表面通透性(PS)间的差异,并以病理为基础初步对BF、BV值与血管内皮生长因子(VEGF)进行相关性研究。结果转移组淋巴结BF值明显高于炎症组(P=0.004),分别为(150.00±70.00)ml/(min.100g)、(75.00±40.00)ml/(min.100g);BV、PS及MTT值与炎症组无统计学差异(P>0.05)。转移组淋巴结的TDC呈速升速降型,炎症组淋巴结的TDC以缓慢上升型为主。两组淋巴结的BF、BV值与VEGF表达呈正相关(r=0.821,P<0.001;r=0.519,P=0.001)。结论 CTPI可在毛细血管水平反映兔淋巴结的血流灌注状况,推测淋巴结内血管生成状态,有助于鉴别良。
Objective To observe the value of CT perfusion imaging(CTPI) in differentiation of axillary inflammatory hyperplasia and metastatic lymph nodes.Methods Fourteen female New Zealand white rabbits were randomly averagely divided into 2 groups,and rabbit models of axillary inflammatory hyperplasia and metastatic lymph nodes with VX2 implanted breast carcinoma were established respectively.All rabbits underwent CTPI.The differences of time density curve(TDC) and CT perfusion parameters including blood flow(BF),blood volume(BV),mean transit time(MTT) and permeability surface(PS) between the two groups were analyzed.On the basis of pathology,the correlation of BF,BV with expression of vascular endothelial growth factor(VEGF) were preliminarily assessed.Results BF value of metastatic lymph nodes(150.00±70.00)ml/(min·100 g) was distinctly superior to that of inflammatory hyperplastic lymph nodes(75.00±40.00)ml/(min·100 g),(P=0.004).There was no statistical difference of BV,PS and MTT between the two groups.TDC of all the metastatic lymph nodes presented as quickly rise and fall type.The main TDC of inflammatory hyperplastic lymph nodes presented as slowly rise type.There were positive correlations between the expression degree of VEGF and BF,BV value of inflammatory hyperplastic and metastatic lymph nodes(r=0.821,P0.001;r=0.519,P=0.001).Conclusion CTPI gives information that reflect the status of blood perfusion on the level of blood capillary of lymph nodes and presumes vascularization of rabbits lymph nodes,being helpful for differentiation of benign and malignant lymph nodes.
