摘要
目的应用扩张器扩张颈肩胸联合皮瓣治疗中重度颏颈胸粘连。方法 2008-03～2011-03间,应用皮肤软组织扩张器扩张的颈肩胸联合皮瓣治疗颏颈胸粘连27例。患者颏部、颈部、胸前瘢痕为中重度,颏颈胸粘连Ⅱ度以上。预扩张颈侧面肩上胸三角区皮肤,扩张2个月后全麻下切除颏颈胸瘢痕,充分松解挛缩,创面大小约为10 cm×13 cm～8.0 cm×12 cm。设计旋转推进皮瓣,转移到颈前覆盖创面,供区直接拉拢缝合。结果全部病例术后效果良好,随访3～18个月,满意率85.18%,颈部活动自如。术后发生血肿2例,经及时行血肿清除术,未影响治疗效果。发生皮瓣远端血运障碍3例,表皮出现张力性水泡,通过间断拆除缝线,术后应用改善血液微循环药物,理疗仪理疗,应用重组人酸性表皮细胞生长因子等措施后,创口愈合,局部有表浅瘢痕形成。结论应用皮肤软组织扩张器扩张颈肩胸联合皮瓣治疗颏颈胸部瘢痕挛缩畸形,能取得较好的疗效。
Objective To study the treatment results of scar around chin,neck and chest scars with expanded flap from cervical,humeral and thoracic flap.Methods From March 2008 to March 2011,we used expanded skin flap from the cervical,humeral and thoracic to repair severe soft tissue defects of the chin,neck and chest on 27 patients.Results Most of the patients have good results.Complications happened including hematoma 2 cases,3 cases of ischemic necrosis is genesis at the distal side of the flap.Post-operation follow-up of to18 months flap,there is no contraction.Conclusion The cervical,humeral and thoracic flap of expanded skin can be satisfactorily applied to repair the severe chin,neck and chest scars.
