摘要
目的探讨家庭延伸护理对老年慢性心力衰竭(chronic heart failure,CHF)患者生存率的影响。方法将108例老年CHF患者随机分为观察组和对照组各54例,对照组按常规方法,出院前由主管护师与患者及家属共同制定出院后详细的家庭护理计划,出院后,由患者及家属实施家庭护理计划,由其主管护师每3个月电话跟踪随访1次,了解患者的情况,持续2年;观察组从入院开始即由经统一培训的主管护师负责,出院后继续由主管护师2次/周电话随访和每2周家访1次实施一对一的家庭延伸护理1年,第2年同对照组由其主管护师每3个月电话跟踪随访1次。比较两组患者出院2年后的生存率。结果出院2年后,观察组患者生存率明显高于对照组(P<0.01)。结论家庭延伸护理可以有效提高老年CHF患者的生存率,值得临床推广应用。
Objective To explore the impact of extended family care on the survival rate of elderly patients with chronic heart failure（CHF）.Methods A total of 108 elderly CHF patients were randomly divided into observation group（n=54） and control group（n=54）.In observation group,the charge nurses who received professional training were responsible for the nursing of patients and after discharge,there were still telephone follow-ups twice per week and home visit once in two weeks which were conducted by charge nurses but in control group,routine nursing was provided and there was no extended family care from nurses after discharge.A comparison was made of the survival rate of patients in two groups two years after discharge.Results The survival rate of patients in observation group was obviously higher than that in control group（P0.01）.Conclusion Extended family care can not only improve compliance behavior after discharge of elderly CHF patients but improve their quality of life and eventually increase the survival rate.
