摘要
目的:比较二维子宫输卵管超声造影(2D-HyCoSy)与三维子宫输卵管超声造影(3D-HyCoSy)对输卵管通畅性的诊断价值。方法:对32例不孕症患者行2D-HyCoSy及3D-HyCoSy,与其腹腔镜结果对照,通过构建受试者应用曲线,比较2种方法对输卵管通畅性的诊断价值。结果:2D-HyCoSy诊断输卵管通畅的敏感性、特异性、阳性预测值、阴性预测值分别为83.3%、100.0%、100.0%、96.3%,准确率89.1%;3D-HyCoSy分别为90.9%、100.0%、100.0%、98.1%,准确率92.2%。构建二者的ROC工作曲线,2D-HyCoSy的AUC为0.981,3D-HyCoSy的AUC为0.991,两者AUC差异有统计学意义(P<0.05)。结论:三维子宫输卵管超声造影图像清晰、客观,能真实展现输卵管的立体空间走形,易于临床判断,具有较好的临床应用前景。
Objective: To compare the value of two-dimensional or three-dimensional hysterosal pingo- contrast sonography (2D-HyCoSy vs 3D-HyCoSy) in the fallopian tubes patency. Methods:32 patients underwent 2D-HyCoSy and 3D-HyCoSy ,the results were compared with those of laparoscopy. The diagnostic value of 2D-HyCoSy and 3D-HyCoSy was evaluated by constructing the receiver operating characteristic curve(ROC). Results: The sensitivity, specificity, positive predictive value and negative predictive value of 2D-HyCoSy in diagnosing patency of fallopian tube were 83.3%, 100.0%, 100.0%, 96.3%, the accuracy was 89.1%;the above data for 3D-HyCoSy in diagnosing patency of fallopian tube were 90.9%, 100.0%, 100.0%, 98.1%, the accuracy was 92.2%. When ROC curve was set up, the AUC of 2D-HyCoSy was 0.981, while AUC of 3D-HyCoSy was 0.991, the difference was significant ( P〈 0.05). Conclusions: 3D-HyCoSy'image is clear,objective and able to perfectly show the shape of uterine tube, can offer more clinical information for the diagnosis.
