摘要 目的:通过与Tc-99m-MDP显像的对比,评价Ga-67显像在淋巴瘤骨转移的临床价值。方法:63例经CT,X线平片或组织活检证实淋巴瘤骨转移病人,17例霍杰金氏淋巴瘤(HL),46例非霍杰金氏淋巴瘤(NHL),所有病人均行Ga-67和Tc-99m-MDP全身骨扫描,其中42例行化疗4个疗程后的Ga-67显像。静脉注射Ga-67370MBq(10mCi)和740MBq(20mCi)的Tc-99m-MDP,分别在48-72小时和3-4小时后,进行前后位全身扫描。结果:63例淋巴瘤骨转移的病人中,共有骨病灶128个。Ga-67显像诊断灵敏度为89%,而Tc-99m-MDP诊断灵敏度为98%;Ga-67显像与Tc-99m-MDP骨扫描对比发现有31个病灶内Tc-99m-MDP摄取高于Ga-67的摄取,而有21个病灶内Ga-67摄取高于Tc-99m-MDP的摄取。Ga-67显像对治疗反应的评价:随访中,在42例治疗前Ga-67骨显像阳性者,Ga-67显像提示完全治愈的准确率为91%;未治愈者,Ga-67显像还有Ga-67异常摄取。结论:67Ga显像可准确地判断淋巴瘤骨转移的疗效;对淋巴瘤骨转移的探测Tc-99m-M? Objective: Evaluation of Ga-67 imaging in lymphoma with bone involvement was performed by the comparison with Tc-99m-MDP bone scan.Methods: In 63 patients with bone involvement of lymphoma proven by CT, X-rays or biopsy included 17 with Hodgkin's lymphoma(NL) and 46 with Non-Hodgkin's lymphoma(NHL), Ga-67 and'Tc-99m-MDP scintigraphy were performed after 48-72h of Ga-67 injection(10mCi) and 3-4h of Tc-99m-MDP injection(20 mCi) in all patients. of these, 42 patients were imaged after 4 cycles of chemotherapy. Results: In 63 patients total bone lesion is 128. The diagnostic sensitivity of Ga-67 and Tc-99m-MDP scintigraphy is 89 % and 98% respectively. The Tc-99m-MDP uptake is higher than Ga-67 in 31 bone lesions,but there are 21 bone lesions with higher Ga-67 uptake than Tc-99m-MDP.About monitoring therapy, accurate rate of Ga-67 imaging is 91%, the hare lesions with no remission were still have ab- normal uptake of Ga-67. Conclusion: Da-67 scintigraphy correctly monitored the response to treatment in patients with bone involvement of lymphoma; Detection rate of Tc-99m-MDP imaging was better than Ga-67 imaging in patients with bone involvement of lymphoma.
