摘要
卵巢过度刺激综合征(OHSS)是体外受精-胚胎移植(IVF-ET)过程中,超促排卵药物的广泛应用而出现的一类严重医源性疾病,目前其发病机制未明,治疗上仅限于对症支持治疗,效果不尽人意。运用传统中医学理论,结合前人经验,对OHSS的病机治法作了初步的探讨。认为OHSS是本虚标实之病,肾之精气亏虚为本,血脉不和、水饮气滞为标。肾之精气受损,失其固摄、推动、气化之职,血脉失和,有瘀血、出血、阴血不足,"血不利则为水",引起水饮停聚,瘀血水饮又引起气机失调,从而出现一系列临床综合征。治疗上以补肾填精、滋阴养血、活血止血为主,行气利水、化痰散结为辅,并注意保养胎元。自拟一方为主,临床上灵活加减,取得了一定的效果。
Ovarian hyperstimulation syndrome（OHSS） is a category of serious iatrogenic complications.In this paper,I try to make a primary exploration in the pathogeny and pathogenesis of OHSS through traditional Chinese medical theory combined with clinical practice.It is considered that the pathogenesis of OHSS is intricate,with the existing overflow from blood veins,the poor blood circulation,the lack for Yin blood,also with internal retention of phlegm and water-pathogen,and the inadequate kidney essence.Among these pathogenesis,Xuefen is the fundamentality,Shuifen is the consequence.The therapeutic principle purpose is circulating blood to stop bleeding,nourishing Yin blood,supplementing fetal vitality.Self-made herbs treatment is achieved and resulted in satisfactory effects.
