摘要
目的研究吸烟对十二指肠溃疡(Duodenal ulcer,DU)的影响。方法无吸烟组和吸烟组并能在8周内成功随访的的患者各50例,经内镜检查确诊为十二指肠溃疡活动期并速尿素酶试验为阳性。均口服兰索拉唑30 mg,克拉霉素500 mg,阿莫西林1 000 mg,每日2次,连续7 d,然后继续兰索拉唑30 mg每日1次,治疗3周。全部治疗结束停药后4周复查内镜并行速尿素酶试验检测。结果无吸烟组和吸烟组幽门螺杆菌(Helicobacter pylori,Hp)根除率分别为90.0%和88.0%;DU愈合率分别为96.0%和86.0%;痊愈率分别为70.0%和40.0%。Hp根除率、愈合率两组间差异均无统计学意义(p>0.05),DU痊愈率无吸烟组显著高于吸烟组(p<0.05)。结论吸烟对Hp根除率及愈合率无明显影响,但会降低十二指肠溃疡痊愈率。
Objective To study the effect of smoking on duodenal ulcer treated with. Methods Non smoking and smoking group and in 8 weeks the success of all 50 patients,endoscopic examination diagnosed as duodenal ulcer activity period and rapid urease test positive.Are oral lansoprazole 30 mg,amoxicillin（1 000） mg,clarithromycin 500 mg,2 times daily,continuous 7d,and then continue to lansoprazole 30 mg 1 times a day,for 3 weeks.All the end of treatment 4 weeks after drug withdrawal.Review of endoscopic parallel speed urease test. Results Non smoking and smoking group of Helicobacter pylori eradication rates were 90.0% and 88.0%,DU healing rates were 96.0% and 86.0%,cure rates were 70.0% and 40.0%.Hp eradication rate,healing rate difference between the two groups were not statistically significant（p0.05）.DU cure rate without smoking group were obviously higher than those of the smoking group（p0.05）. Conclusion The effect of smoking on Hp eradication rate and healing rate had no significant effect,but will reduce the duodenal ulcer healing rate.
