Influence of average heart rate and heart rate variability on image quality using 64-section CT coronary angiography

摘要 目的研究心率快慢与心律不齐对64排CT冠状动脉造影图像质量的影响。方法对接受64排CT冠状动脉造影患者84例行前瞻性研究,其中女26例,男58例;平均年龄(65.37±10.44)岁。在扫描R-R间期内以5%间隔重建图像。心率变化程度以扫描期间各心率的标准差进行评价。图像质量分为四级(1级无运动伪影,-4级不能诊断)。对心率快慢及心律变化程度与图像质量的关系行Pearson相关分析。结果扫描时平均心率为(74.63±11.33)次/min,心律变化为(3.52±2.41)次/min。1 836个节段中有1 821个节段为可诊断图像。右冠状动脉与前降支图像质量与心率快慢无相关性。左旋支图像质量随着心率的增快而变差(r=0.384,P<0.05)。心律不齐程度对图像质量有明显影响(r=0.710,P<0.001)。在心率>80次/min时,最佳重建时相由舒张期改为收缩期。结论 64排CT可在高心率条件下提供可诊断图像,但适当控制心率,可以明显改善64排CT冠状动脉成像质量。 Objective To investigate the influence of average heart rate and heart rate variability on image quality using 64-section computed tomographic(CT) coronary angiography.Methods There were 84 patients(26 women,58 men;mean age,65.37±10.44 years) who underwent 64-section CT coronary angiography with retrospective electrocardiographic gating.Data sets were reconstructed in 5% steps within the exposure windows.Heart rate variability was calculated as standard deviation during scanning.Image quality was evaluated by 4-point method(1: no motion artifacts to 4: can not be evaluated).Pearson correlation analysis was performed to compare image quality with average heart rate and heart rate variability.Results Mean heart rate was 74.63±11.33 beats per minute(bpm),with variability of 3.52±2.41 bpm.Diagnostic image quality(score3) was attained in 1821 of 1836 segments after optimal reconstruction.There was no correlation between mean heart rate and image quality for all segments of the right coronary and left anterior descending arteries,but there was a significant correlation for left circumflex artery(r=0.384,P0.05).Heart rate variability was correlated with image quality overall(r=0.710,P0.01).The optimal reconstruction intervals shifted from diastole to systole when heart rate80 bpm.Conclusion 64-section CT can provide diagnostic image quality within a wide range of heart rates.However,average heart rate and heart rate variability is suggested to reduce artifacts.
