摘要
目的:分析晚期早产儿围生期危险因素及并发症发生情况,为防治提供依据。方法:分析和比较137例晚期早产儿与329例足月儿临床资料和围生期并发症的发病情况。结果:Apgar评分、出生体重、住院天数、母体围产期高危因素方面两组新生儿均存在统计学差异(P<0.05);研究组新生儿黄疸(92.0%)、低血糖低体温(31.3%)、呼吸系统疾病(17.5%)、喂养不耐受(8.8%)的发生率明显高于对照组(83.59%、6.99%、2.43%、0),P<0.05;研究组新生儿合并多项并发症85.40%的发病率明显高于对照组19.45%(P<0.05)。晚期早产儿并发症的发生率按顺位排列为:黄疸92.0%,心血管系统疾病33.6%,低血糖低体温31.3%,呼吸系统疾病17.5%,颅内出血及缺氧缺血性脑病16.8%,感染性疾病14.6%,喂养不耐受8.8%。结论:Apgar评分较低和母体围产期高危因素都可能成为晚期早产儿出现并发症的危险因素,临床应加大对高胆红素、心电图、血糖的监测力度,提早发现问题,及早治疗。
Objective:To analyze the risk factors and complications of late preterm infants,provide a basis for prevention and treatment. Methods:The clinical data and perinatal complications of 137 late preterm infants and 329 term infants were analyzed and compared. Results:There was significant difference in Apgar score,birth weight,hospitalization time and maternal high risk factors during perinatal period between the two groups(P0.05).The incidences of neonatal jaundice(92.0%),low blood glucose and low temperature(31.3%),respiratory system diseases(17.5%) and feeding intolerance(8.8%) in study group were significantly higher than those in control group(83.59%,6.99%,2.43%,0)(P0.05).The incidence of multiple neonatal complications in study group was 85.40%,which was significantly higher than that in control group(19.45%)(P0.05).The incidences of complications among late preterm infants from high to low were listed as follows: jaundice(92.0%),cardiovascular system diseases(33.6%),low blood glucose and low temperature(31.3%),respiratory system diseases(17.5%),intracranial hemorrhage and hypoxic-ischemic encephalopathy(16.8%),infectious diseases(14.6%) and feeding intolerance(8.8%). Conclusion:Low Apgar score and maternal high risk factors during perinatal period may become risk factors of complications among late preterm infants,the monitoring of high bilirubin,electrocardiogram and blood glucose should be enhanced in clinic for early detection and timely treatment.
