摘要
目的探讨超声引导下经皮、经肝胆囊穿刺引流术(PTGD)治疗高龄高危急性胆囊炎的疗效。方法回顾性分析我院2006年2月～2009年6月应用PTGD治疗62例高龄高危急性胆囊炎患者的临床资料。结果 62例老年急性胆囊炎患者中,穿刺置管成功61例,成功率为98.39%,患者发热、寒战、右上腹疼痛、腹膜炎等症状迅速缓解,感染中毒症状得到有效控制1,例穿刺置管失败,行急诊手术治疗,全组无死亡病例。结论对于高龄高危急性胆囊炎患者合并有心肺功能障碍等众多内科疾病,急诊手术治疗具有极高风险,选择PTGD治疗高龄、高危急性胆囊炎是一项安全、简便、有效的方法。
Objective To study the therapeutics of acute cholecystitis in elderly high-risk patients,it is used that ultrasound-guided percutaneous and PTGD.Method Clinical data of 62 cases of acute cholecystitis in elderly patients with high risk was retrospective analysed which was applicated with PTGD treatment in February 2006 ~ June 2009 of our hospital.Results Of the 62 cases of acute cholecystitis in elderly patients with high risk,61 cases of successful cannulation,the success rate was 98.39%,fever,chills,right upper quadrant pain,peritonitis was quickly relieved,symptoms of infection was under control.One case failed,underwent emergency surgery,there was no death case in the group.Conclusion For the high risk elderly patients with acute cholecystitis,pulmonary dysfunction,and many other medical conditions,emergency surgical treatment has a very high risk,PTGD treatment is a safe,simple and effective method against elderly,high risk of acute cholecystitis.
