摘要
目的探讨颅脑损伤患者血小板参数的变化及其临床意义。方法对85例颅脑损伤患者分别于伤后1、3、7、14、21d采集静脉血,测定血小板数量(platelet count,PLT)、血小板比积(platelet pressure accumulates,PCT)、血小板平均体积(mean platelet volume,MPV)和血小板分布宽度(platelet distributionwidth,PDW)值,回顾性分析颅脑损伤与其关系。同时测定50例健康体检者PLT、PCT、MPV和PDW作为对照。结果与健康对照组相比,伤后1 d,PLT、PCT、MPV和PDW无明显变化(P>0.05),伤后3 d,PLT和PCT明显降低(P<0.01),而MPV和PDW明显升高(P<0.01)。与伤后1d相比,伤后3d时PLT和PCT明显降低(P<0.01),而MPV和PDW无明显变化(P>0.05)。伤后7~21 d,重型颅脑损伤患者PIT和PCT明显高于轻、中型颅脑损伤患者(P<0.01),而MPV和PDW则明显低于轻、中型颅脑损伤患者(P<0.01)。结论密切观察颅脑损伤患者血小板参数的变化有助于其病情判断和预后评估。
Objective To study the changes in platelet parameters in patients with acute craniocerebral trauma and their clini- cal significance. Methods The platelet count(PLT) , platelet pressure aceumulates(PCY) , mean platelet volume (MPV) and plate- let distribution width(PDW) in venous blood were detected 1, 3, 7, 14 and 21 days after injury in 85 patients with craniocerebral trau- ma. The relationship between platelet parameters and craniocerebral trauma were retrospectively analyzed. The PLT, PCT, MPV and PI)W in 50 healthy subjects served as controls. Results The PLT, PCT, MPV and PDW 1 day after injury were similar to those in control group (P 〉 0.05 ). Compared with control group, the PLT and PCT decreased significantly (P 〈 0.01 ) while MPV and PVW increased significantly (P 〈 0.01 ) 3 days after injury. Three days after injury, the PLT and PCT decreased significantly (P 〈 0.01 ) while MPV and PVW were similar to those ( P 〉 0.05 ) 1 day after injury. Compared with the group of mild to moderate craniocerebral trauma, the PLT and PCT increased significantly (P 〈 0.01 ) while MPV and PDW decreased significantly (P 〈 0.01 ) in the group of severe craniocerebral trauma from 7 to 21 days after injury. Conclusions Close observation of the changes in platelet parameters helps the judgment of condition and prognosis in patients with craniocerebral trauma.
