摘要
目的:了解和分析我院抗菌药物使用现状,提出抗菌药物应用管理的对策。方法:(1)从医院HIS系统收集抗菌药物的总量和费用及临床科室抗菌药物使用率。(2)随机抽取2010年出院病历365份进行回顾性调查,记录患者基本情况和抗菌药物的使用情况,作为临床合理用药的评价依据。结果:抗菌药物费用占医院总药费的百分比为21.3%;住院患者抗菌药物使用率为60.4%;所抽365份病历中,接受抗菌药物治疗患者的抗菌药物平均品种数为1.6个;接受抗菌药物治疗患者的抗菌药物日平均费用为197.0元;使用单种抗菌药物治疗的平均天数为8.9 d;手术患者接受预防感染抗菌药物百分比为86.1%;手术患者接受预防感染抗菌药物天数为6.8 d;接受抗感染治疗的患者,送病原学检查百分比为55.3%;抗菌药物不合理使用平均每份病历1.1处。结论:抗菌药物管理的指标有所好转,但仍偏高,存在不合理用药,应加大对抗菌药物应用的综合治理,降低抗菌药物费用,提高抗菌药物使用的合理性。
OBJECTIVE：To probe into the status quo of the antibiotic use of in our hospital and put forward countermeasures for management on clinical use of antibiotics.METHODS：The data of consumption sum and quantity of the antibiotics as well as the rate of antibiotic use in clinic were collected from Hospital HIS system.A total of 365 discharged medical records in 2010 were randomly collected for retrospective analysis of patients＇ general data and use of antibiotics on based on which,the rationality of clinical use of antibiotics was evaluated.RESULTS：The cost of antibiotics accounted for 21.3% of the total.Antibiotics were used in 60.4% of the inpatients.For the 365 cases,an average of 1.6 antibiotics was used among the antibiotic receivers.The daily average antibiotic cost was 197.0 Yuan per antibiotic receiver.Each antibiotic was used for an average of 8.9 days.86.1% of the patients who have undergone surgery received prophylactic antibiotics for 6.8 days.Etiological test was done for 55.3% of the patients who received anti-infective therapy.Each medical record has 1.1 improper uses of antibiotics on average.CONCLUSION：The index of antimicrobial management improved to some degree yet it is still on the high side and somewhat irrational.Therefore,measures should be taken to intensify the comprehensive management on antibiotic use to lower the cost on antibiotics and improve the rationality of antibiotic use.
