大鼠急性脊髓完全性损伤后血脊膜屏障通透性的变化

摘要 目的:探讨大鼠急性脊髓完全性损伤后血脊膜屏障(blood-spinal cord barrier,BSCB)通透性的变化。方法:本研究选用wistar大鼠48只,制作Allen氏脊髓打击模型(打击当量300gcf),随机分为对照组(n=6)和脊髓损伤组(n=42),后者又根据伤后不同时间点分为2、8、24、48、72h以及1、2周组,每组6只,再分为A组和B组,每组各3只。A组从大鼠股静脉注射2%的伊文思蓝(Evens blue,EB),通过荧光显微镜下观察;B组用干湿重法测定脊髓水肿变化情况。结果:随着脊髓损伤时间的延长,脊髓组织EB蓝染的范围逐渐增大,荧光光斑数量增加,强度增强,24~48h达高峰,与脊髓组织内水肿时间一致,72h后水肿逐渐减轻,其内开始出现坏死灶,1~2周可见明显大小不等的囊腔形成,未见EB蓝染。结论:脊髓损伤后BSCB的通透性增高,并在24~48h达高峰,72h后逐渐下降,1周后BSCB通透性明显减小。 Objective To study the changes of blood-spinal cord barrier (BSCB) permeability after acute complete spinal cord injury in rats. Methods Forty-eight healthy wistar rats were used in this study. The Allen's spinal cord droping model was induced by 300gcf. There were one control group of 6 rats and seven spinal cord injury groups of 42 rats which were assigned to subgroups of 2 h, 8 h, 24 h, 48 h, 72 h, 1 w and 2 w(the time of post- injury). The spinal cord injury groups were further assigned to A and B groups according to diffirent time, each of which contained 3 rats. A groups : 2% solution of Evans blue was administered intravenously from vena femoralis of rats in a dose of 4 mL/kg at expected time after SCI. Then the BSCB permeability was evaluated with fluorescence microscope. B groups : Dry-humid weight method was observed the changes of edema of spinal cord. Results With the prolonging of the spinal cord injury's time, the scope of EB blue-staining in spinal cord tissue increased gradually, and the number and area of fluorescent faculae increased till 24-48h,with one accord of spinal cord edema's time. And then it stepped down gradually till 72 h after SCI, meanwhile, to appeared cellular necrosis. Obviously to form inequality of size of capsular space of spinal cord after injury later lweek and 2week groups, and there was not seen the blue fluorescence of EB. Conclusion The increasing of BSCB permeability after SCI, and to reach the peak during the 24-48h, and then it stepped down gradually till 72 h. It is evidently the permeability of BSCB to diminish after SCI one weeks.
