急进高原个体外周血氧自由基变化与急性胃肠粘膜损伤分析 被引量:4

摘要 目的对急进高原个体外周血氧自由基变化与急性胃肠粘膜损伤情况分析。方法对20例由平原急进高原个体分别于进入高原前1 d、进入高原第3 d和第7 d进行胃镜检查,并同步抽取血液标本,对其外周血清丙二醛(MDA)、过氧化氢(H2O2)的含量进行检测。结果 20例平原个体急进高原后第3d及第7d胃镜检查阳性例数分别为17例(约85%)和10例(50%),与进入高原前比较差异有统计学意义(P<0.01),主要表现为胃粘膜出血、充血、淤血、糜烂、溃疡等胃粘膜损伤。MDA、H2O2与进入高原前比较,除MDA第7 d P>0.05外,其余均显著升高,以第3 d表现明显(P<0.01)。结论高原急性缺氧可导致个体急性胃肠粘膜损伤,并且跟其缺氧后的氧自由基显著升高存在明显的关系。故对于急进高原胃肠道症状明显者,应给予胃肠道粘膜保护剂和氧自由基清除剂,这对减轻急进高原者胃肠道粘膜的损伤和降低急进高原者急性高原病的发病率具有积极的意义。 objective Analysis changes of oxygen radicals in peripheral blood and injury of gastric and intestinal mucosal.Methods take examination of gastroscopy for 20 people who entered into plateau rapidly in 1 day before they entered into plateau and 3,7 days after they entered into plateau separately,then draw blood and detected the content of malondialdehyde and H2O2 in peripheral blood serum at the same time.Result there were 17 cases in 3 days and 10 cases in 7 days showed positive results according to gastroscopy,and has significant diversity compare with that was in plain(P〈0.01),the main manifestation of gastroscopy were bleeding congestion and ulcer of stomach mucosal.Except MDA showed no dramatica difference between plain and plateau groups,MDA and H2O2 both showed significant increase in plateau groups compare with that of plain group,especially in 3 days group(P〈0.01).Conclusion acute expose of hypoxia in plateau can cause injury of gastric and intestinal mucosal,and degree of injury is correlated closely to the increase of oxygen radiations led by hypoxia.So we should give protecting agents and oxygen radiations scavengers to patients with obvious digesting symptoms caused by acute high altitude exposed,and this has positive sense for reducing the damage of gastric and intestinal mucosal and prevalence of acute high altitude diseases.
