摘要
目的分析食管食源性骨性异物穿孔多层螺旋CT表现,提高对食管异物穿孔诊断及并发症的认识。方法搜集行CT检查的食管异物患者资料45例,并结合X线表现进行回顾性对比分析。结果 X线表现:①食管穿孔14例,大块骨异物35例,小块骨异物8例,未发现异物2例。②CT表现:食管穿孔45例,食管周围炎31例,食管脓肿4例,食管-气管瘘2例,食管-纵隔瘘3例,吸入性肺炎5例,气胸1例。结论多层螺旋CT采用VR、MIP等后处理技术是诊断食管食源性骨性异物穿孔的首选方法,对发现食管小骨块异物穿孔等并发症的检出率显著高于食道造影检查,具有很高的敏感性。
Objective To analyze MSCT performance of esophageal food-borne osseous eyewinker perforation,and to improve the cognition of esophageal perforation diagnosis and the complications.Methods The collected data of 45 patients with esophageal eyewinke for CT examination were compared and analyzed retrospectively in combination of X-ray performance.Results ①X-ray performance:14 cases of esophageal perforation,35 cases of large bone eyewinker,8 cases of small bone eyewinker,2 cases of no eyewinker.② CT performance:45 cases of esophageal perforation,31 cases of esophageal surrounding inflammation,4 cases of esophageal abscess,2 cases of esophageal-tracheal fistula,3 cases of esophageal-mediastinum fistula,5 cases of aspiration pneumonia,and one case of pneumothorax.Conclusions Post-processing technology such as VR,MIP applied in MSCT is the preferred method for diagnosis of esophageal food-borne osseous eyewinker perforation,which has significantly higher detection rate of small bone eyewinker perforation and other complications than esophageal imaging due to its high sensitivity.
