摘要
目的:探讨并改良膝关节镜下应用羟基磷灰石界面螺钉治疗胫骨平台骨折SchatzkerⅢ型骨折术式的临床效果。方法:将18例胫骨平台SchatzkerⅢ型骨折患者采用关节镜监视下对塌陷移位的骨折进行复位,恢复关节面的平整,打压填塞植骨,羟基磷灰石界面螺钉支撑固定,同时处理合并伤。结果:所有患者术后均无创口或关节感染、皮肤坏死、创口不愈合或骨筋膜室综合征出现,骨折均获解剖复位或近解剖复位,优16例,良2例。结论:关节镜下使用羟基磷灰石界面螺钉治疗胫骨平台SchatzkerⅢ型骨折,具有切口小,直视使骨折关节面达到解剖复位并可再次调节复位高度,固定可靠,避免日后复位高度丢失,无需另行切口经皮螺钉固定及C臂机透视,无需2次手术拆除内固定,可同时处理合并的关节内损伤的优点。
Objective：To evaluate the clinical effect of schatzker Ⅲ tibial plateau fracture fixed with screw coated with hydroxyapatite under arthroscope.Methods：Schatzker Ⅲ tibial plateau fractures in 18 patients were displaced and the articular surface were restored under arthroscope,bone defect were filled with bone chips and were compacted,fractures were fixed with screw coated with hydroxyapatite.Results：wound infection,joint infection,skin necrosis and psteofascial compartment syndrome were not occurred in all of the patients,fractures were anatomically or nearly anatomical replaced.16 cases were excellent,2 cases were good.Conclusion：Schatzker Ⅲtibial plateau fracture fixed with screw coated with hydroxyapatite under arthroscope,with small incision,had the following advantages,such as replaced anatomically,the height could be reset again,no loss of height someday,no further invasive percutaneous screw fixation,no C-arm X-ray machine and no necessary to removal the internal fixation for secondary surgery,and the intra-articular injury can be treated at the same time.
