摘要 目的:应用动态血糖监测系统(CGMS)研究临床2型糖尿病肾病患者血糖波动状态。方法:应用CGMS对30例2型糖尿病肾病(DN)患者和15例单纯2型糖尿病(DM)患者进行连续3d的动态血糖监测。结果:DN组与单纯DM组相比,其MBG、MIN、>7.8AUC及PT、>11.1AUC及PT和MODD均显著大于对照组(P均<0.05);DN组较对照组的中、晚餐血糖水平显著升高(P均<0.05),DN组具有较明显的餐后高血糖。结论:DN患者的平均血糖水平较单纯DM水平高,高血糖持续时间长,日间血糖波动性大,处于慢性波动性高血糖状态,餐后明显高血糖。应用CGMS进行动态血糖监测能使临床医生详细地了解2型DN患者血糖波动的细节。 Objective:Explore the blood glucose fluctuation in the patients with type 2 diabetic nephropathy(DN)by the continuous glucose monitoring system(CGMS).Methods:Thirty type 2 diabetic patients with clinical diagnosed diabetic nephropathy and 15 type 2 diabetic patients without complication underwent continuous glucose monitoring for 3 days(72 h)by CGMS.Results:The 24-hour mean blood glucose(MBG),MIN,7.8 area under curve(AUC)and percentage of time(PT),11.1 AUC and PT as well as mean of daily difference(MODD)were significantly increased in the group of diabetic nephropathy,compared with the group of DM-2 without complication(all P0.05).The study showed that the blood glucose level of the patients with diabetic nephropathy fluctuates throughout the day.Conclusion:The mean blood glucose in the patients with diabetic nephropathy is higher than those of DM-2 without complications,with the characteristics of long-lasting high BG period,dramatic instability during the day and especially high postprandial blood glucose.
