摘要
目的探讨入院24h平均血糖水平对重症脓毒血症患者病情的判断及预后的意义。方法入选162例重症脓毒血症患者,入院即刻开始每2h测定血糖,计算血糖均值,按入院24h平均血糖水平分成5组:<4.5mmol/L,4.5~7.7mmol/L,7.8~10.0mmol/L,10.1~12.0mmol/L,>12.1mmol/L,采用单因素方差分析比较各组患者7d死亡率及存活者APACHEⅡ评分。结果以4.5~7.7mmol/L作为对照组,10.1~12.0mmol/L和>12.0mmol/L死亡率和APACHEⅡ评分与对照组比较有统计学差异。结论平均血糖水平过高是脓毒血症预后不良的危险因素。
Objective To evaluate the impaction of first 24 hours mean blood glucose(MBG) level on the prognosis of severe sepsis. Methods This observational analysis enrolled 162 severe sepsis patients,Plasma glucose was measured at admission and every two hours after admission,The MBG level through the first 24hours for each patient was calculate.Patients were stratified into five groups according to their MBG levels:4.5,4.5~7.0,7.1~10.0,10.1~12.0,12.0mmol/l.The incidence of mortality and APACHE Ⅱ score of every level were analyzed at the seventh day.Results Compared with the MBG of 4.5~7.7mmol/L group, The 7-day mortality and APACHE Ⅱ score of 10.1~12.0mmol/L and 12.0mmol/L group increased.Conclusion The higher mean blood glucose level is the bad prognosis for severe sepsis patients.
