摘要
目的探讨胃肠道间质瘤(GIST)组织中Maspin、uPA、VEGF和VEGF-C的表达及临床意义。方法应用免疫组织化学方法和逆转录PCR法检测GIST组织中Maspin、uPA、VEGF及VEGF-C的表达情况,并分析各指标表达与GIST生物学行为之间的关系以及Maspin与uPA、VEGF、VEGF-C表达的相关性。结果 42例GIST组织中,Maspin、uPA、VEGF和VEGF-C蛋白的表达率分别为61.9%(26/42)、64.3%(27/42)、59.5%(25/42)和54.8%(23/42)。逆转录PCR检测显示,Maspin、uPA、VEGF和VEGF-C mRNA在GIST组织中均有表达,且与其相应蛋白表达的趋势基本一致。随着GIST侵袭危险性增加,Maspin表达显著降低,uPA蛋白及VEGF、VEGF-C表达均显著增加(P<0.05),而uPA mRNA表达无显著性差异(P>0.05)。随着GIST出现复发转移,Maspin表达显著降低,VEGF和VEGF-C表达显著增加(P<0.05),而uPA表达无显著性差异(P>0.05)。Maspin与uPA、VEGF、VEGF-C表达均呈显著负相关(r=-0.429,P<0.05;r=-0.312,P<0.05;r=-0.292,P<0.05)。结论 Maspin表达下调或缺失促进uPA、VEGF和VEGF-C表达上调在GIST演进过程中可能扮演重要角色,4种蛋白均有可能是预测GIST生物学行为及预后的重要辅助因子;Maspin基因有可能是GIST靶向治疗新的研究靶点。
Objective To investigate the expression and clinical significance of maspin,uPA,VEGF and VEGF-C in gastrointestinal stromal tumor(GIST) patients.Methods The expression of maspin,uPA,VEGF and VEGF-C were detected by immunohistochemistry and RT-PCR,respectively.The relations between the expression of each factor and biological behavior and the correlation among maspin and uPA,VEGF as well as VEGF-C were respectively analyzed.Results In 42 GISTs tissues,the protein expression rates of maspin,uPA,VEGF and VEGF-C were 61.9%(26/42),64.3%(27/42),59.5%(25/42)and 54.8%(23/42),respectively.RT-PCR indicated that the mRNA of maspin,uPA,VEGF and VEGF-C also distributed extensively in GIST groups and were coincident with the protein changes.With the increase of the invasion risk,the positive expression of maspin was reduced and the positive expression of uPA protein,VEGF and VEGF-C protein were increased(P0.05).The uPA mRNA expression did not show obvious difference(P0.05).The positive expression of maspin statistically decreased and that of VEGF and VEGF-C significantly increased with recurrence or metastasis(P0.05).uPA expression had no significant relation with recurrence or metastasis(P0.05).There were significant negative correlation between the expression of maspin and that of uPA,VEGF and VEGF-C,respectively(r=-0.429,P0.05;r=-0.312,P0.05;r=-0.292,P0.05).Conclusion Reduced or absent expression of maspin promotes up-regulation of uPA,VEGF and VEGF-C expression,which might play an important role in tumor progression of GIST.And maspin,uPA,VEGF and VEGF-C will be important factors in predicting GIST biological behaviour and judging prognosis,and maspin gene may be a new target for GIST targeted therapy.
