摘要
目的探讨多层螺旋CT(MSCT)首过期灌注成像在肺部肿瘤中的应用价值。方法通过MSCT动态增强首过期灌注值及强化峰值来描述不同类型肺部肿瘤的灌注特征。选取30例肺部肿瘤患者进行检查。注射对比剂同时屏气,进行同层动态增强扫描,1.0s为一个周期,进行30个周期的扫描。通过对感兴趣区域的分析来评价其时间密度曲线,计算灌注值及强化峰值,从不同方面进行比较。结果直径>4cm的肺部肿瘤灌注值及强化峰值明显低于直径≤4cm的肿瘤,两者差异有统计学意义(P<0.05);除周围型肺癌与单侧转移瘤灌注值及强化峰值之间差异无统计学意义外,其余各型肺部肿瘤之间两两比较,其灌注值和强化峰值差异均有统计学意义(P<0.01);非小细胞肺癌与小细胞肺癌两两比较,其灌注值和强化峰值差异均无统计学意义(P>0.05)。结论肿瘤灌注值及强化峰值与肿瘤大小位及置有关,而与病理类型无关。
Objective Discussed the application value of the MSCTfirst expired perfusion imaging in lung neoplasm.Methods To discuss perfusion features of different types lung neoplasmwith dynamic CTperfusion.30 patients with lungneoplasmselected forinspection.Duringbreath hold,afterinjection of contrastmedium,30 slices scan were performed(1 scan/s) at a fixed table position.Through the analysis of the interested region to evaluate its time density curves,and reinforcementof peak value calculation infusion fromdifferentaspects.Results Perfusion and peak enhancementvalue of large tumors(diameter4 cm) was significantly lower than that of smaller ones(diameter≤4 cm).Perfusion value and strengthening peak of central lung neoplasm and peripheral lung neoplasmhad significant difference.Perfusion value and strengthening peak of central lung neoplasmand unilateral metastases had significant difference.Perfusion value and strengthening peak of peripheral lungneoplasmand unilateral metastases had no significantdifference.There was no significantdifference in both parameters between non-small cell lungcancers(NSCLC)and small cell lungcancers(SCLC).Conclusion Tumor perfusion and peak enhancement value depend on tumor size and localization,but not on histology.
