摘要
目的探讨多发性骨髓瘤(MM)肾病患者的肾脏彩色多普勒超声特征。资料与方法选取多发性骨髓瘤患者91例,将血肌酐水平<133μmol/L的55例纳入MMⅠ组,血肌酐≥133μmol/L的36例纳入MMⅡ组,选取无心、肾疾病的25例志愿者作为对照组。观察双肾大小、实质厚度、段动脉和叶间动脉收缩期最大流速(PSV)和阻力指数(RI)并在3组之间进行比较。结果 MMⅡ组双肾长度和实质厚度均小于对照组(P<0.05)和MMⅠ组(P<0.05),MMⅡ组双肾缩小和实质回声增强检出率均高于对照组和MMⅠ组(P<0.05),MMⅠ组实质回声增强检出率高于对照组(P<0.05)。MMⅡ组双肾段动脉及叶间动脉PSV低于对照组及MMⅠ组(P<0.001),双肾段动脉及叶间动脉RI均高于对照组及MMⅠ组(P<0.05),MMⅠ组叶间动脉RI高于对照组(P<0.05)。结论多发性骨髓瘤肾病患者肾脏彩色多普勒特征为实质回声增强,肾脏稍缩小,段动脉和叶间动脉PSV降低及RI升高。
Purpose To investigate color Doppler ultrasonography characteristics of multiple myeloma with nephronia.Materials and Methods Ninety one patients with multiple myeloma and twenty five healthy controls underwent renal color Doppler ultrasonography for kidney length,parenchymal width,peak systolic velocity and resistive index of segmental and interlobar renal arteries.Based on serum creatinine laboratory analyses multiple myeloma patients were divided into normal group(group 1,n =55) and abnormal group(group 2,n =36).Results Group 2 patients showed smaller kidney length and parenchymal width than that in group 1(P 〈 0.05) and control group(P 〈 0.05),and a higher incidence of renal atrophy and parenchyma hyperechogenicity than that in group 1(P 〈 0.05) and control group(P 〈 0.05).The incidence of renal parenchyma hyperechogenicity in group 1 was significantly higher than that in control group(P 〈 0.05).Lower peak systolic velocity and increased resistive index of segmental and interlobar renal artery were found in group 2 than that in group 1 and control group(P 〈 0.05) with increased resistive index of interlobar renal artery in group 1 than that in control group(P 〈0.05).Conclusion Multiple myeloma with nephronia shows a higher incidence of renal parenchyma hyperechogenicity on color Doppler,slightly smaller length of renal,lower peak systolic velocity of segmental and interlobar renal artery,and increases resistive index of segmental and interlobar renal arteries.
