摘要
目的初步探讨64层螺旋CT(MSCT)冠状动脉造影中体重、心率、体重指数(BMI)、每分心输出量(CO)、射血分数(EF)对对比剂增强效果的影响及其关系。方法 (1)对160例临床疑似冠心病患者行CT冠状动脉造影(CTCA)检查,测量患者升主动脉根部的平均CT值。将体重、心率、BMI、CO与升主动脉根部平均CT值进行逐步回归分析;(2)体重、CO与升主动脉根部平均CT值行Pearson相关分析;(3)对60~70 kg体重组患者心率与升主动脉根部平均CT值作Pearson相关分析。结果逐步回归方程为Mean(升主动脉根部平均CT值)=829.962-5.324 Weight+5.253BMI-1.648 Heart rate-0.013CO(F=48.253,P=0.000);由标准回归系数可见,对升主动脉根部平均CT值的影响从大到小依次为体重(-0.851)、BMI(0.209)、心率(-0.173)、CO(-0.169)。体重与升主动脉根部平均CT值呈高度负相关(r=-0.665,P〈0.01);CO与升主动脉根部平均CT值呈低度负相关(r=-0.355,P〈0.01);60~70 kg体重组心率与升主动脉根部平均CT值呈低度负相关(r=-0.268,P〈0.05)。结论 (1)在相同对比剂使用条件下,患者体重是影响CTCA增强效果的重要因素,二者呈负相关,即体重越大,冠脉增强幅度越小;BMI、CO以及心率(控制在80次/min以下时)对冠脉增强效果影响较小;EF对增强效果无显著影响。(2)配合合理的冠脉扫描技术,可根据体重适当调整对比剂的剂量。
Objective: To explore the influence factors on enhanced effect of contrast material in 64 multi-slice spiral CT coronary angiography: weight,heart rate,BMI,CO and EF.Methods:(1) 160 patients suspected CAD underwent CT coronary angiography using 64 multi-slice CT(60ml contrast materials at 5.0ml/s).The attenuation values in Hu were measured on the foot of the ascending aorta.The body weight,heart rate,BMI,CO,EF and the attenuation of the ascending aorta were analysed by multiple linear regression analysis.(2) The body weight,CO and the attenuation of ascending aorta were assessed with Pearson correlation analysis.(3) The heart rate and the attenuation of ascending aorta were assessed with Pearson correlation analysis in the group of 60 to 70kg.Results: The result of multiple linear regression analysis was: Mean=829.962-5.324 Weight+5.253BMI-1.648 Heart rate-0.013CO(F=48.253,P=0.000).The influence from high to low was: weight(-0.851),BMI(0.209),heart rate(0.173) and CO(-0.169).A significant negative correlation was found between weight and attenuation(r=-0.665,P0.01).CO and the attenuation of ascending aorta had low negative correlation(r=-0.355,P0.01).Similarly,a significant low negative correlation was found between the heart rate and the attenuation of ascending aorta in the group of 60 to 70kg(r=-0.268,P0.05).Conclusion:(1) Provided the same dosage of contrast material,weight is the critical influence factor in enhanced effect of contrast material,following by BMI,heart rate,and CO.There is a significant negative correlation between weight and enhanced effect,while there is no correlation between EF and enhanced effect.(2) The dosage of contrast material can be reduced according to weight,combined with reasonable scan technology.
