摘要
目的建立巨结肠动物模型,研究肠间质细胞(interstitial cells of Cajal,ICC)在不同肠段的分布情况,探求ICC与巨结肠的病因的关系。方法取8~9周龄SD大鼠90只,随机分为两组。氯胺酮麻醉下开腹,实验组用0.1%苯扎氯铵(BAC)处理大鼠降结肠浆膜40 min,温盐水冲洗后关腹,对照组用生理盐水代替BAC。分别于术后1、2、3、4、8周进行大体观察、钡灌肠,结肠测压、取处理段结肠行病理学检查,并通过免疫组化SABC法观察实验大鼠不同肠段ICC的分布情况。结果在正常对照组和动物模型组有神经节细胞段结肠壁内ICC的分布情况大致相同。在动物模型组无神经节细胞段肠壁内ICC数目明显减少(P<0.05)。肌间神经节周围ICC形成的网络状结构遭到严重破坏,肌层内ICC减少更为显著甚至消失。结论采用BAC实验方法建立的动物模型模拟了临床上巨结肠的相应变化,在动物模型组病变肠段神经节细胞的缺如或减少与ICC的变化的一致性,证实ICC介导了肠段蠕动功能。
Objective To establish a rat model of experimental aganglionosis,and to investigate the distribution of interstitial cells of Cajal(ICC) in different segments.Methods Ninety Sprague Dawley rats,8~9 weeks old,were randomly divided into two groups: treatment group(n=45) and control group(n=45).All animals were operated upon under Ketamine anesthesia.In treatment group,0.1% benzalkonium chloride(BAC) was applied onto the serosa for 40 min,and then the colon was washed well with 0.9 % normal saline.For the control group,0.9% saline was applied using the same method.Macroscopic and microscopic pathologic observations of colon,colon manometry,Barium enema examination were performed at an interval of 1 week,2 weeks,3 weeks,4 weeks and 8 weeks after operation and the way of immunohistochemisty was used to investigate the distribution of ICC in different segments of a rat model of experimental aganglionosis.Results In the segments of a rat model of experimental aganglionosis,compared with control group and the ganglionic segments group,ICC were decreased obviously of aganglionic segments group(P0.05).The numbers,morphous and distribution of ICC were different in each segments.In most aganglionic segments,the ICC was absence,the relic ICC got blunting,decreased or disappeared foot process.Conclusion The animal model for experimental aganglionosis provides an experimental basis for future research regarding relationship between ICC and etiology of Hirschsprung′s disease(HD).The variation of ICC in different segments of colon in rat model is in coincidence with that of ganglion cell.The abnormal distribution,numbers and morphous of ICC may be part of the cause to gastrointestinalmotility disorder in HD.
