摘要
目的探讨先天性巨结肠(HD)肠壁S-100蛋白的表达和神经元型一氧化氮合酶(nNOS)阳性神经的异常分布。方法对25例HD的病变肠段行S-100蛋白和nNOS染色,比较其染色结果和分布特点。结果扩张段及狭窄段结肠壁S-100在神经丛内的神经纤维、施万细胞及周围细胞均有阳性表达,而在神经节细胞则无阳性表达;扩张段nNOS阳性神经节细胞和神经纤维形态分布良好,狭窄段无nNOS阳性神经节细胞,散在分布细小nNOS阳性神经纤维以黏膜层为多。扩张段及狭窄段中神经纤维数密度无明显差异(t=1.251 P>0.05),但前者神经纤维直径明显较后者大(t=4.492 P<0.01)。结论神经分布异常是HD发病的机制之一,S-100与nNOS的协同检测可作为HD明确诊断的重要手段。
Objective To explore the expression of soluble protein 100(S- 100) and abnormal distribution of neuronal nitric oxide synthase(nNOS) protein in the colon of Hirschsprung's disease(HD). Methods Intestinal biopsy of 25 cases with HI) was made with SP immunohistochemical staining method. The characteristic expressions of S - 100 and nNOS were observed and compared. Results Nerve fiber,schwann cell and peripheral cell of expanding segment were S= 100 positive, but neurons were S- 100 negative, nNOS positive; There were no nNOS positive neurons in stricture segment, but. nerve fiber was nNOS positive, which distributed more in the mucous layer, while in the expanding ,segment, nNOS positive neuronsdistributed regularly as normal morphous. There was no significant difference in the density of nerve fiber between expanding segment and stricture segment( t = 1. 251 P 〉 0.05). The diameter of nerve fiber in the expanding segment was bigger than stricture segment( t = 4. 492 P 〈 0.01 ). Conclusions The abnormal nerve distribution is one of the major physiological mechanisms of HD. Detection S- 100 combined with nNOS is a significant way in the diagnosis of HD.
