摘要 目的:对照研究骨关节炎(OA)中软骨磁共振(MRI)表现与主要临床症状的相关性,探讨常见的OA临床症状与MRI表现之间的关系。方法:征集40~70周岁确诊的OA病例56例,采用以下序列行软骨MR成像:①快速自旋回波T2加权成像(FSE T2WI);②三维脂肪抑制扰相梯度回波序列(3D-FS-SPGR)及最大信号强度重建(MIP);③脂肪抑制质子加权序列(FS-PD)。分别比较软骨缺损状况和软骨下骨质水肿情况,并将磁共振表现与临床症状进行相关性分析。结果:OA患者膝关节软骨缺损平均Noyes IIIA级以上,可观察到不同程度的软骨缺损及软骨下骨质异常;软骨下骨质水肿与膝关节疼痛部位相关,但软骨缺损与疼痛部位无相关性。结论:MRI可见的软骨下骨质异常与关节疼痛部位相关,软骨缺损不影响OA症状的表达。 Purpose: To compare the magnetic resonance imaging(MRI) appearances of osteoarthritis (OA) with the clinical findings. To assess the value of MRI in clinical diagnosis of OA and explore the correlation between cartilage MRI and clinical findings. Methods: Fifty - six cases of 40 - 70 years old patients with confirmed OA and 45 asymptomatic age matched volunteers were included. They were scanned with cartilage MR in the following sequences: Fast spin echo T2 -weighted imaging(FSE T2WI); 3D Fat - suppressed spoiled gradient echo sequence(3D - FS - SPGR) and maximum intensity projection reconstruction(MIP); Fat - suppressed Proton - weighted sequence(FS - PD) . The following features were compared: (i)The average thickness of cartilage; (2)The defect condition of cartilage; (3)Subchondral bone edema, then the correlation between MRI Appearances and clinical findings were analyzed. Results: Knee articular cartilage defects of the articular surface area were involved in the patient group more than the control group(Z = - 6.23, P 〈 0. 001) ; The Noyes level and the range of cartilage defects of patients group were significantly higher than that of the control group(Z = - 8.336, P 〈 0.001; Z = - 8.42, P 〈 0.001) ; Subchondral bone edema was associated with knee pain sites, but the cartilage defects were not associated with the pain. Conclusion: The articular cartilage of 40 - 70 years old patients with knee OA is significantly thinner than the asymptomatic same age group, and the cartilage defects of patients group are significantly higher than the control group. The abnormal subchondral bone area on MRI is associated with joint pain, meanwhile the cartilage defects do not affect the occurrence of the symptoms.
