早期肠道喂养改善烧伤后肠道血液灌流的实验研究 被引量:27

Intestinal blood perfusion improved by early enteral feeding in burned rat

摘要 目的 探讨早期肠道喂养改善烧伤大鼠肠道血液灌流的机理。 方法 采用30 % 体表面积Ⅲ度烧伤大鼠模型,分为正常对照( C) 、单纯烧伤( B) 和早期喂养( E F) 组。分别检测伤前及伤后3 、6 、12 、24 、48 小时肠组织中内皮素( E T) 、一氧化氮( N O) 、血管活性肠肽( V I P) 含量及肠粘膜血流量( I M B F) 。 结果 烧伤后肠组织中 E T、 N O、 V I P及 E T/ N O、 E T/ V I P 比值均呈上升趋势,而 I M B F 则显著低于伤前。 E F 组中 E T 含量及 E T/ N O、 E T/ V I P 比值明显低于 B 组,而 N O、 V I P 及 I M B F 则高于 B 组, E T/ N O、 E T/ V I P同 I M B F呈显著负相关(r1 = - 095 , P< 001 ;r2 = - 087 , P< 001) 。 结论 早期肠道喂养改善烧伤后肠道缺血状况的机制可能与食物对肠壁神经的刺激导致 N O、 V I P 释放增加, E T含量降低有关。 Objective To explore the mechanism of intestinal blood perfusion improved by early enteral feeding after burn injury. Methods Wistar rats were inflicted with 30% total body surface area full thickness flame burn and randomly divided into three groups: burned control(B), burned and early feeding(EF), and normal control(C). The content of endothelin(ET), nitric oxide(NO),vasoactive intestinal peptide(VIP), and intestine mucosal blood flow(IMBF) were determined at postburn 0,3,6,12,24,48 hours. Results The content of ET,NO,VIP in small intestine as well as the ratio of ET/NO,ET/VIP were increased significantly and the IMBF was decreased markedly postburn. In EF group, ET, ET/NO and ET/VIP were significantly lower( P <0 05, P <0 01), and NO,VIP as well as IMBF were higher than those in B group( P <0 05, P <0 01). There were negative correlation between ET/NO, ET/VIP and IMBF( r 1=-0 95, P <0 01; r 2=-0 87, P <0 01). Conclusions The mechanism of intestinal blood perfusion improved by early enteral feeding may be correlated with increased NO,VIP and decreased ET induced by foods stimulating intestinal nerve.
