摘要
目的:观察中药益气活血方对糖尿病肾病患者血、尿Ⅳ型胶原和层黏蛋白的影响,方法:将入选60例患者随机分为治疗组和对照组,每组各30例,健康人群组来之我院体检中心和我院职工。治疗组和对照组组均给予控制血糖、血脂、血压、尿蛋白以及抗凝等常规治疗,治疗组加用益气活血中药(黄芪、丹参、山药、茯苓、桃仁、蜈蚣、白术、川牛膝);观察2组治疗前后的血、尿Ⅳ型胶原和层黏蛋白变化。结果:健康组与治疗组、对照组治疗前比较有着显著差异(P<0.05),治疗组与对照组治疗前比较无差别(P>0.05,治疗组治疗前后比较检测的四项指标均有统计学意义,其中血IV胶原(P<0.05)、血层黏连蛋白(P<0.01)、尿IV胶原(P<0.05)、尿层黏连蛋白(P<0.05);对照组治疗前后比较检测的四项指标均有统计学意义,其中IV胶原(P<0.05)、血层黏连蛋白(P<0.05)、尿IV胶原(P<0.05)、尿层黏连蛋白(P<0.05),治疗组与对照组治疗后比较仅有尿层黏连蛋白统计学意义(P<0.05);结论:中药益气活血方可能通过调节血、尿Col-IV、LN等,在延缓肾脏纤维化方面具有一定的疗效。
Objective： To study the effect of TCM Treatment with principle of supplementing-Qi nourishing-Yin and activating blood circulation on serum,urinary collagen Ⅳ（Col-Ⅳ）,laminin（LN） with diabetic nephropathy.Methods： 60 cases were divided into treatment group（30 cases） and control group（30 cases） randomly.The members of healthy group are from the physical examination center of our hospital and staff of our hospital.The conventional treatment of controlling blood glucose,blood lipid,blood pressure,urinary protein and anticoagulation were given to the treatment group and control group at the same time.For the treatment group,patients were treated with Chinese herbal medicine with effects of replenishing qi and activating blood additionally（Astragalus,Dashen.Yam,Tuckahoe,Semen Persicae,Centipede,Atractylodes macrocephala,radix cyathulae）;to observe the change of serum Col-IV,urinary Col-IV and lanimin of two groups after treatment.Results： Compared to the treatment group and control group before the treatment,the healthy group showed obvious difference（P0.05）;compared to the control group before treatment,the treatment group showed no obvious difference（P0.05）;after treatment,the four indicators are of significant difference,namely,serum collagen Ⅳ（Col-Ⅳ）（P0.05）;serum LN（P0.05）,urinary Col-Ⅳ（P0.05）;urinary LN（P0.05）;compared to the control group after treatment,the treatment group showed great difference on urinary LN（P0.05） only.Conclusion： The principle of supplementing-Qi nourishing-Yin may have some healing effect in counterbalancing renal fibrosis by the way of ajusting blood,urineCol-IV,LN and so on.
