摘要
目的探讨胃癌组织中M2型巨噬细胞和调节性T细胞(Tregs)的表达情况及其联合检测对预后的预测价值。方法收集1999年2月至2005年12月期间复旦大学附属中山医院行D2根治术并达到R0切除的135例胃癌患者临床及随访资料。将手术切除标本制作组织芯片,以CD163作为M2型巨噬细胞标记,以Foxp3作为Tregs细胞标记,免疫组织化学染色后,分别以阳性细胞密度和阳性细胞计数来表示M2型巨噬细胞和Tregs细胞的表达。结果肿瘤组织中M2巨噬细胞密度和Tregs阳性细胞计数中位数分别为7.48/HP和6.33/HP.高于癌旁组织的1.37/HP和2.92/HP(均P〈0.01)。胃癌组织内M2型巨噬细胞与调节性T细胞表达呈显著正相关(r=0.415.P〈0.01)。M2和Tregs均低表达的43例患者中位生存期为99.0个月,显著长于M2和Tregs均高表达的45例患者(72.3个月,P〈0.05)。结论联合检测M2型巨噬细胞和Tregs表达这两种不同来源的抑制性免疫细胞可有效预测胃癌预后。
Objective To explore the prognostic value of M2 macrophages and regulatory T cells (Tregs) in gastric carcinoma. Methods Clinicopathological characteristics and follow up data of 135 patients with gastric carcinoma were collected. Patients included were those who underwent D2 radical resection (RO) at Zhongshan Hospital of Fudan University from February 1999 to December 2005. Tissue chips of gastric carcinoma specimen were stained using immunohistochemistry to determine the cells density and number of M2 (CD163 positive) and Tregs (Foxp3 positive). Results The median positive cells density of M2 macrophages and Tregs in tumor tissue were 7.48/HP and 6.33/HP, respectively, higher than that in adjacent tissues (1.37/HP and 2.92/HP, P〈0.001). The density of M2 macrophages was positively correlated with that of Treg cells (r= 0.415, P〈0.001) in tumor tissue. The median survival of patients with low expression of M2 and Tregs (n=43) was significantly longer than those with high expression of the 2 cells (n=45) (99.0 vs. 72.3 months,P〈0.05). Conclusion Combined detection of M2 macrophages and Tregs may predict the prognosis of gastric carcinoma.
