摘要
目的探讨静脉留置针在血液病病人使用中更有效妥当的固定方法,以确保留置针的有效留置时间及减少其留置期间并发症。方法将临床622例使用静脉留置针的病人随机分为观察组和对照组,两组严格按照留置针无菌操作规程进行穿刺,穿刺成功后,观察组先用3M无菌敷贴固定在针眼上,再应用3M自粘弹力绷带在3M无菌敷贴外沿着肢体平整稍加压地缠绕2圈,轻轻按压绷带即可自行粘牢。对照组单纯选用3M无菌透明敷贴固定留置针,其外未使用弹力绷带再固定。分别观察两组病人留置针留置时间及其留置相关并发症情况。结果观察组留置针在限定的留置时间内未出现脱落,留置时间明显长于对照组(P<0.05);观察组留置期间其并发症也明显少于对照组(P<0.05)。结论 3M自粘弹力绷带应用于血液病人外周静脉留置针中能起到有效妥善的固定,在限定的时间内使留置针更好地发挥了保留的作用,减少了多种并发症的发生,减轻了病人的痛苦;同时也减少了因留置针脱落而引发的医疗纠纷及其他严重不良事件,减轻了病人及医院的经济负担,且此方法取材方便、操作简单、经济适用,值得广泛推广。
Objective To investigate the proper usage of peripheral venous detaining needle for hematological patients in order to ensure the detaining time and reduce complications. Methods Totally 622 cases treated with peripheral venous detaining needle were randomized into intervention group and control group. The sterile detaining needle punctuation procedure was carried out strictly for all the patients. After punctuation, in intervention group, detaining needle was firstly fixed with 3M sterile dressing and then 3M self-adhesive elastic bandage was used outside the 3M sterile dressing around the extremity. In control group, only 3M sterile dressing was used. The detaining time and com- plications of the two groups were observed. Results The detaining needle did not fall off during certain time-period in intervention group and the detaining time was prolonged than that in control group（P 〈 0. 05 ）. Meanwhile, the incidences of various complications were reduced in intervention group compared with those in control group （P 〈 0. 05 ）. Conclusion Application of 3M self-adhesive elastic bandage in hematological patients treated with peripheral venous detaining needle can ensure the proper fixation, prolong detaining time, reduce the incidence of complications and alleviate the pain of patients. Besides, it can avoid medical disputes caused by falling-off of detaining needle. Furthermore, this method is cheap and simple, thus is worthy of promotion.
