摘要
目的 探讨磁共振血管造影( M R A) 对门静脉高压症( P H T) 患者门静脉系统的成像价值。 方法 实验组15 例 P H T患者,对照组为10 例正常人; M R A 参数: Gd D T P A02 m mol/kg 肘静脉注射,3 D F I S P序列扫描门静脉系统, M I P 法重建血管; P C 法测定门静脉流向、流速、流量。 S T A T A 软件对结果进行非配对t 检验。 结果 (1) 门静脉高压症患者门静脉主干( M P V) 、脾静脉( S P V) 、肠系膜上静脉( S M V) 直径、 M P V/ A O 分别为(158 ±017) cm 、(128 ±017) cm 、(110 ±019) cm 、095 ±010 ,都明显大于对照组(113 ±010) cm 、(090 ±010) cm 、(080 ±019) cm 、066 ±010 。(2) Child A、 B 级患者间 M P V、 M P V/ A O 分别为(153 ±021) cm 、(162 ±017) cm 、096 ±012 、095 ±020 ,两组无差别。(3) P H T患者的曲张静脉、侧支循环显影良好。 结论 M R A
Objective To investigate the value of MRA in imaging the portal venous system of portal hypertensive patients. Methods The portal vein system of 15 portal hypertensive patients (PHT) and 10 controls were compared with magnetic resonance angiography (MRA). Methods:Gd DTPA 0 2 mmol/kg was used as contrast,3D FISP sequence was used to image portal venous system, maximum intensity projection (MIP) method to reconstruct vessels, phase contrast (PC) method to measure the flow velocity,amount and direction. Results Main portal vein 〔MPV (1 58±0 17) cm〕,splenic vein 〔SPV (1 28±0 17) cm〕,superior mesentery vein 〔SMV (1 10±0 19) cm〕 and MPV/AO (0 95±0 10) were larger in PHT than those of control respectively 〔(1 13±0 10) cm,(0 90±0 10) cm,(0 80±0 19) cm,0 66±0 10〕.The differences of MPV,MPV/AO between Child A and B patients were not significant.Positive imaging cases of variceal veins in PHT were:Coronary in 15,esophageal and upper esophageal branch in 10,gastric fundus in 13,esophageal varix in 10.Conclusions MRA is superior to Doppler and other routine procedures in imaging portal venous system in portal hypetensive patients.
