摘要
目的评价a-2b干扰素（Interferonc a-2b，IFNd a-2b）与5-氟尿嘧啶（5-Fluorouracil，5．Fu）用于青光眼滤过手术的抗瘢痕效果。方法采取随机对照临床试验研究方法，将52例（56只眼）青光眼分为两组。每组各有28眼，其中4例为同一患者左右眼对照；患者年龄50-75岁，均为初次手术。术中分别一次性使用含500万u的IFN a-2b与浓度为25mg／mL5-Fu，共5min。术后必要时结膜下注射。随访时间4个月。结果术后与术前相比：两组眼压均明显降低（P〈0．001）；5-Fu组术后视力下降者11眼（39．3％），而IFNct-2b组下降3只眼（10．7％）。两组间差异有统计学意义（Y2：6．095，P〈0．05）；IFNct-2b组形成的滤过泡以Ⅱ型为主，5一Fll组则以I型为主（X^2=8．096，P〈0．05）；IFNa-2b组角膜上皮缺损发生率显著低于5一Fu组（P〈0．05）。结论IFN a-2b应用于青光眼滤过术，减少滤过手术失败的作用与5-Fu相当，且并发症较少。根据各自的优缺点，建议两者联合应用或选择应用。
Objective To compare the results of interferon a-2b （IFNct-2b） with 5-Fluorouracil （5-Fu） in glaucoma filtering surgery. Methods 52 cases （56 eyes） glaucoma were randomly studied by treatment with tFN a-2b or 5-Fu following trabeculectom, each group consisting of 28 eyes, one eye was de signed to use IFN a-2b, and the other eye with 5-Fu. All patients were ranged from 50 to 75 years old, and re ceived the first surgery. The IFNa-2b treated eyes and the 5-Fu receiving eyes were intraoperatively adminis tered respectively with 5X10^5 IU and 25 mg/ml via a sponge under the eonjunctival fla for 5 minutes. The follow up was for 4 months. Results Comparing the postoperative with preoperative, the intraocular pres sure （IOP） of the two groups significantly declined （P 〈0.01 ）. The decrease of visual acuity was 39.3% in 5-Fu group ,whereas only 10.7% in IFN a-2b group. The difference between the two groups was significant （X^2 =6. 095,P〈0.05）. The eyes with IFN a-2b tended to form type 1I blebs were according to Kronfeld classification,whereas type I blebs were commonly seen in 5-Fu treated eyes （X^2 = 8.096,P 〈0. 05 ）. The rate of corneal reaosion in IFNct-2b was significantly lower than that in 5-Fu group （ P 〈 0.05 ）, other side effect in IFN a-2b geroup was littler than 5-Fu. Conclusions IFN a-2b has similar effect as 5-Fu to reduce the risk of failure of glaucoma filtering surgery, but more advantages over because of its fewer complications.
